  • Expertise in prompting and configuring large language models, including Function Calling and Context Caching.
  • Experience integrating large language models into platform and application workflows, including experience with MCP protocols.
  • Expert-level experience designing and implementing retrieval-augmented generation systems using embeddings and vector databases.
  • Strong SQL skills for BigQuery.
  • Experience with vector databases such as Pinecone, Milvus, or Vertex AI Vector Search.
  • Mastery of Python and FastAPI.
  • Experience with Docker and containerization for deploying microservices to Cloud Run or Google Kubernetes Engine.
  • Familiarity with scalable, enterprise-level distributed systems.
  • Experience with advanced data structures, algorithms, and complexity analysis.
  • A demonstrated history of product value delivery.
  • Five or more years of software development experience.
  • Two or more years of hands-on experience building and deploying enterprise-grade agentic applications, such as applications using LangGraph or LangChain.
  • A bachelor's degree in Computer Science, Computer Engineering, or a closely related quantitative field is required.
  • Candidates must be U.S. citizens, Green Card holders, or currently in the United States with a valid H-1B visa.
Responsibilities
  • Architect and implement agentic workflows.
  • Build stateful autonomous control loops in which agents interpret clinical data, plan necessary checks, and generate risk assessments.
  • Build a conversational, chatbot-style user experience powered by large language models.
  • Leverage Vertex AI for model serving and large-language-model reasoning, optimizing for long-context windows to process complex medical histories.
  • Implement enterprise grounding using Vertex AI Vector Search so agent outputs are cited against official medical literature, including medical research and guidelines.
  • Build robust Python Function Calling tools that allow agents to securely query BigQuery and other data stores for patient demographics and symptoms.
  • Ensure AI operations comply with HIPAA and GDPR by implementing personally identifiable information masking and data governance in Google Cloud.
  • Deploy agents on Vertex AI, Cloud Run, or Google Kubernetes Engine for low-latency inference and high availability for clinical users.
  • Implement an AI-agent execution monitoring and alert system to support service-level agreement, safety, and security compliance.
  • Guide a team of engineers in MLOps best practices, including continuous integration and continuous delivery for models and evaluation pipelines.

Desired Qualifications
  • Experience with the GCP Vertex AI ecosystem.
  • Familiarity with handling unstructured medical text or clinical guidelines.
  • A master's degree in Artificial Intelligence, Machine Learning, or a relevant field.

C the Signs uses artificial intelligence to help healthcare professionals detect cancer earlier by analyzing patient symptoms, clinical data, and risk factors in real time to suggest diagnostic and referral pathways. It integrates into primary care and broader health system workflows, translating complex cancer guidelines into practical decisions across all cancer types, including rare ones. The platform supports faster referrals, reduces diagnostic delays, and enables more personalized cancer care while focusing on population health to identify disparities and align with national targets. Its goal is to improve patient outcomes by enabling earlier diagnosis and scalable, data-driven cancer care across health systems.

What believers are saying

  • March 2026 Essex pathway expands C the Signs across high-miss upper GI cancers.
  • January 2026 NHS news showed Northamptonshire ICB rolling C the Signs into every GP practice.
  • $8 million Khosla-backed U.S. expansion funds American commercialization and regulatory adaptation.

What critics are saying

  • NHS procurement concentration makes one stalled ICB rollout damage revenue immediately.
  • A negative NHS randomized evaluation would freeze national expansion and kill U.S. sales.
  • Epic, Cerner, and diagnostics vendors can bundle similar triage, compressing margins.

What makes C the Signs unique

  • UKCA-marked, NHS-born workflow embeds cancer triage directly into primary care.
  • North East Essex launched England's first AI case-finding pathway for oesophageal and gastric cancers.
  • Independent evaluation tied Suffolk and North East Essex deployment to 272 earlier diagnoses.

NHS launches first AI-driven early detection pathway for oesophageal and stomach cancers

A UK-first, AI-enabled pathway has launched in North East Essex to improve early detection of oesophageal and stomach cancers, which are among England's hardest cancers to diagnose early. Around four in five cases are currently diagnosed at advanced stages. Delivered by AstraZeneca, C the Signs, NHS Suffolk and North East Essex ICB, Health Innovation East and GP Primary Choice, the programme uses AI to analyse electronic health records and identify at-risk patients. Eligible patients receive SMS invitations to complete digital assessments, with high-risk cases triaged to appropriate diagnostics including capsule sponge testing or endoscopy. C the Signs already supports over 1,500 GP practices across England. The programme aligns with the National Cancer Plan's ambition to shift from reactive to predictive, risk-based approaches for cancers without established screening programmes.

C the Signs Raises $8M for U.S. Expansion

C the Signs, an AI-powered cancer detection company, raised $8M in a funding round led by Khosla Ventures to expand from the UK to the US. The company's platform analyzes EHRs to identify cancer risk early, improving survival rates. In the UK, it identified 20.7% of breast cancer cases up to five years earlier and improved early-stage ovarian cancer diagnosis by 53.3%. The funding aims to enhance early detection and personalized care in the US healthcare system.
