Requirements
  • A bachelor's degree in recreation and leisure studies, therapeutic recreation, kinesiology, or an equivalent field is required; a Bachelor of Social Work and Registered Social Worker designation in Ontario are assets.
  • At least three years of post-qualification experience in program planning and delivery in a health care setting is required; long-term care knowledge is an asset.
  • Knowledge and experience in counseling with seniors and their caregivers are required.
  • Ability to supervise, coach, and mentor a team.
  • Demonstrated initiative, problem-solving, and judgment skills.
  • Creativity and innovation in developing programs and activities that maximize residents' independence.
  • Strong customer service skills.
  • Ability to contribute effectively and work as part of an interdisciplinary team.
  • Computer literacy is required; knowledge of Chinese word processing is an asset.
  • A satisfactory Police Vulnerable Sector Screen result is required.
  • Fluency in English and Chinese is required.
  • Successful applicants must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.
Responsibilities
  • In consultation with the Executive Director, plan, develop, and ensure delivery of meaningful therapeutic, recreational, and social programs for long-term care residents in accordance with Ministry of Health and Long-Term Care standards and requirements.
  • Support staff in implementing innovative and therapeutic programs and evaluate their effectiveness.
  • Coach, develop, monitor, and evaluate staff while promoting a strong, effective, engaged, and efficient team; encourage open communication and feedback.
  • Assist in developing the internal budget and implement an effective expenditure-control system for the activation program.
  • Ensure efficient and responsible use of resources.
  • Coordinate with interdisciplinary team members to implement and monitor resident care plans related to activation, including charting and documentation.
  • Ensure implementation of risk-management and quality-improvement programs and maintain a safe, hygienic environment for clients and staff.
  • Participate in and contribute to residents' team conferences.
  • Coordinate the optimal deployment of volunteers and provide guidance, training, and support; manage and evaluate volunteer performance and provide feedback.
  • In consultation with the Executive Director, develop, update, communicate, and implement activation-program policies.
  • Ensure current program records and reports, including accurate statistics and evaluations.
  • Provide support to the Residents' Council, including coordinating meetings and taking accurate minutes.
  • Provide backup to the Social Worker as required.
  • Coordinate production of monthly resident and family newsletters.
  • Coach and prepare residents for special functions and programs arranged and initiated by the Yee Hong Community Wellness Foundation.
  • Keep up to date with developments and best practices in the field and implement activation programs reflecting those values and principles.
  • Work occasional weekends or evenings as required.
Desired Qualifications
  • Knowledge of long-term care.
  • A Bachelor of Social Work and Registered Social Worker designation in Ontario.
  • Knowledge of Chinese word processing.
  • Knowledge of Japanese, Filipino, or a South Asian dialect.
