OWN CORRESPONDENTS September 11, 2026 6 Mins Read IN the health zones around Mongbwalu and Bunia, in Ituri province, the arithmetic of this Ebola outbreak has become almost impossible to hold in the mind. A confirmed case is rarely just a patient. It is a household, a lineage of mourners who washed and touched a body before anyone knew what killed it, a clinic worker who has not left an isolation ward in weeks, a child now sleeping in a stranger's home because both parents are gone. Since the Democratic Republic of Congo's Ministry of Public Health declared its seventeenth Ebola outbreak on 15 May 2026, the virus has moved faster and killed more people in a shorter span than almost any Ebola epidemic ever recorded - and it is doing so inside one of the most violent, most neglected corners of the continent. By early September, the Congolese health ministry had confirmed 6,686 cases and 3,226 deaths, with more than 800 patients still fighting the virus in isolation wards. Only the West African epidemic of 2014 to 2016, which killed over 11,000 people across Guinea, Liberia and Sierra Leone, has claimed more lives. In terms of sheer velocity, this one has no precedent: roughly 5,000 cases accumulated in barely one hundred days, a pace the World Health Organisation's own director-general, Tedros Adhanom Ghebreyesus, has warned could eventually eclipse West Africa's death toll altogether. A VIRUS MEDICINE WAS NOT BUILT FOR Part of what makes this outbreak so lethal is the pathogen itself. Unlike the Zaire ebolavirus that drove the 2018-2020 Kivu outbreak and the West African epidemic before it, this one is caused by the Bundibugyo species - rarer, less studied, and historically just as deadly, with case fatality rates in past outbreaks ranging between 30 and 50 percent. Crucially, the monoclonal antibody treatments and the licensed rVSV-ZEBOV vaccine that transformed Ebola response over the past decade were built for the Zaire strain. Against Bundibugyo, they carry no regulatory approval and, according to the WHO's own technical advisers, insufficient evidence of protection. That has left responders relying on the oldest tools in outbreak medicine - supportive care, rehydration, isolation, contact tracing and safe burial - while a scramble for new countermeasures unfolds in real time. In late July, Oxford University enrolled its first volunteer in the world's first clinical trial of a vaccine candidate built specifically against Bundibugyo virus. Moderna has begun early trials of its own mRNA candidate. The WHO authorised a Phase 3 trial of the existing Ervebo vaccine in late July on the strength of animal data suggesting partial cross-protection, and by mid-August was negotiating with Congolese authorities to get a multi-arm vaccine trial running in the field within weeks. None of it changes the calculus for the thousands of families burying relatives this month: for this outbreak, there is still no shot to offer them. A WAR THAT WILL NOT PAUSE FOR A PLAGUE Ituri province, the outbreak's epicentre and home to 82 percent of all confirmed cases, is not merely under-resourced. It is contested territory. The Allied Democratic Forces, a Ugandan Islamist militia aligned with the Islamic State, has carried out repeated massacres in the province; more than 170 people were killed in armed-group violence in Ituri in May alone, the same month the outbreak was declared. CODECO factions, organised along ethnic lines, control swathes of rural territory outside state reach. To the south, in North and South Kivu, the Rwanda-backed M23 alliance holds Goma and large parts of the countryside, even as a Washington-brokered accord and a Doha peace framework technically bind Kinshasa and Kigali to a ceasefire. More than 2,100 people have been killed in that supposedly pacified east since the June 2025 agreement was signed. Over 120 armed groups now operate across the region. Response teams built their protocols for a health crisis. They are operating inside a war. Nearly one million people have been displaced from their homes in Ituri by conflict, scattering exactly the contact networks that epidemiologists need to trace. Doctors and nurses have fled overwhelmed, sometimes abandoned, health facilities. On 4 June, in Katana - a South Kivu town under the control of the Congo River Alliance/M23 - a burial team attempting to safely inter an Ebola victim was confronted by residents who seized the body and buried it themselves, exposing mourners to the very fluids the team had come to contain. Thirteen days later, in the Mongbwalu health zone, another burial team was attacked. On the same day as the Katana incident, eleven Ebola patients walked out of isolation facilities in Ituri, disappearing back into communities where health workers can no longer reliably follow them. The grief itself is part of the danger. In much of the Ituri and Kivu region, funerals can stretch across several days and involve washing, dressing and physically embracing the dead before burial - rites that carry deep spiritual weight and, with a haemorrhagic virus, lethal risk. Aid agencies describe communities caught between fear of the disease and fear of losing the right to mourn their own dead on their own terms, a tension that has repeatedly boiled over into violence against the very teams trying to protect them. THE CHILDREN LEFT BEHIND The epidemiology tells only part of the story. Save the Children has documented more than 300 child deaths in this outbreak by early August, and aid workers warn the deeper wound may be invisible on any case count: children who watch a parent die inside an isolation tent, then face suspicion and exclusion from the community that raised them. In the 2014-2016 West African epidemic, more than 30,000 children lost one or both parents to the virus. Humanitarian officials fear this outbreak, unfolding in a region where roughly 15 million people already needed humanitarian assistance before the first case was confirmed, could leave a comparable scar on a generation already living through displacement, hunger and war. Nearly ten million people across Ituri, North Kivu, South Kivu and Tanganyika are facing acute hunger, according to the latest food security assessments - part of a wider crisis in which roughly a quarter of the DRC's population, some 27 million people, now live with crisis-level food insecurity or worse. Ebola has not arrived in a vacuum. It has arrived on top of the hunger, on top of the guns, on top of a health system that was already, by aid agencies' own description, in a catastrophic state before the first patient in Bunia fell ill in early May. HOLDING THE LINE Despite the odds, the response has not collapsed. More than 1,700 Médecins Sans Frontières staff are working across five DRC provinces, running treatment centres from Mongbwalu to Komanda to Niania and reporting, as of 1 September, more than 1,400 survivors - proof that even against a strain with no vaccine, early supportive care saves lives. In Bunia, a local radio journalist has taken to the airwaves daily to fact-check rumours about the virus and connect frightened listeners to health specialists, a small but telling example of Congolese communities organising their own defence where international response plans fall short. The WHO has scaled up surveillance, cross-border preparedness with Uganda - which declared its own outbreak and briefly sealed its DRC border in late May - and community engagement teams working health zone by health zone. A TEST THE WORLD KEEPS FAILING TO LEARN FROM Every major Ebola epidemic of the past decade has taught the same lesson: the virus is rarely the hardest problem to solve. Distrust, conflict, poverty and the collapse of basic services are. The DRC's seventeenth outbreak is now the clearest test yet of whether that lesson has actually been absorbed - by Kinshasa, by the humanitarian system, and by a Rwanda-backed insurgency that shows little sign of loosening its grip on the very provinces where the outbreak burns hottest. For now, the count of the dead climbs faster than the count of the vaccinated, and in Ituri's health zones, communities continue doing what they have always done in the absence of anyone else able to help: burying their own, and hoping the next funeral is not their own.
