Opportunity Information

Sutter SeniorCare PACE is seeking an experienced Internal Medicine, Family Medicine, or Geriatrics physician to join our interdisciplinary team in Sacramento.The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive care to medically complex older adults with the goal of helping participants remain safely and independently in their communities.This is a unique opportunity to practice relationship-based medicine with low daily visit volume, extended appointment times, broad clinical resources, and a highly collaborative care team.

Practice Highlights

• Typically 7 to 8 patient visits per day, with some days as few as 3
• 90-minute new participant assessments and 60-minute comprehensive reassessments
• Monday through Friday outpatient practice
• Daily interdisciplinary team meeting from approximately 8:00 to 9:00 a.m.
• First patient generally scheduled at 9:00 a.m. and last patient by approximately 3:30 p.m.
• Approximately 625 participants currently served across three care teams
• Each care team includes a physician and nurse practitioner supported by an interdisciplinary group representing 11 disciplines
• Limited traditional patient-portal inbox burden
• Opportunities to participate in quality improvement, memory care, palliative care, polypharmacy reduction, fall prevention, and chronic disease initiatives

The program is expanding its care-team structure to support continued growth and maintain a manageable participant panel.

Comprehensive, Team-Based Care

PACE physicians care for the whole patient, combining primary care with functional, cognitive, behavioral, and social support.Physicians collaborate closely with nursing, social services, pharmacy, physical and occupational therapy, dietary services, home care, and other disciplines. Resources include an onsite physical therapy department and gym, Adult Day Health Center, pharmacy support, psychiatry, and home-based services.Physicians also have access to Sutter specialists across Northern California, with support for specialty scheduling and participant transportation. PACE uses PACE Care Online as its primary EMR, with access to Epic for Sutter specialty and hospital records.Because PACE coordinates participants' healthcare services, physicians can focus on making clinically appropriate decisions with less of the traditional authorization burden associated with complex outpatient care.

Call

Call is shared across the clinical team and supported by a nurse triage service.

• Approximately 1 in 7 to 8 weekday calls
• Approximately 1 in 7 to 8 weekend calls, subject to team coverage
• Weeknight call is approximately 5:30 p.m. to 7:30 a.m.
• Weekend call runs Friday evening through Monday morning
• Call is taken from home
• Nurse triage handles calls using established protocols before escalating physician-level issues
• Late-night calls are uncommon

Qualifications

• MD or DO
• Board certified in one of the following:
  • Internal Medicine
  • Family Medicine
  • Geriatrics
• Minimum five years of physician experience due to the clinical complexity of the participant population
• Commitment to a 1.0 FTE position
• Strong interest in caring for medically complex older adults
• Ability to work effectively within an interdisciplinary, team-based model
