Responsibilities
  • Promote HealthySteps within the practice to families and providers.
  • Conduct team-based well-child visits before, during, or following the primary care provider, focusing on monitoring development, social-emotional functioning, and relational health and providing anticipatory guidance.
  • Conduct short-term consultations for developmental and behavioral concerns such as sleep, positive parenting, and picky eating.
  • Maintain a family support line and respond to and track call requests within the designated response time.
  • Participate in reflective supervision meetings.
  • Conduct home visits and accompany families to key medical, specialty, and community agency appointments as needed.
  • Facilitate parent and caregiver groups as needed.
  • Create and maintain a community resource directory or database.
  • Provide referrals and track follow-up to help families connect with key community resources.
  • Reach out to community providers within the first six months to share information about HealthySteps, support referrals and warm handoffs, and ensure referral criteria are met.
  • Participate in community-wide early childhood and mental health meetings when appropriate.
  • Actively participate in the HealthySteps Implementation Team and attend regular team meetings.
  • Coordinate with front office administrative staff to schedule the HealthySteps Specialist’s time with provider visits.
  • Engage in Continuous Quality Improvement initiatives to ensure the HealthySteps model is delivered with fidelity.
  • Collaborate with the HealthySteps implementation team to implement all eight Core Components, including universal screenings, positive parenting and early learning guidance, and workflow adjustments.
  • Maintain open communication with all members of the primary care team.
  • Work closely with pediatric primary providers on care coordination, goal setting, coaching, and education about child development.
  • Provide consultation and facilitate training for medical professionals and practice staff on early childhood development, early relational health, infant and early childhood mental health, and trauma-informed practice.
  • Maintain databases required to meet HealthySteps fidelity metrics, including internal and external referrals.
  • Collaborate with the HealthySteps Implementation Team to complete required annual site reporting to the HealthySteps National Office at ZERO TO THREE, including analysis of external database files and electronic health record data reports when needed.
  • Document patient clinical activity and care coordination in the electronic health record.
  • Track caseload capacity within the risk-stratified service delivery model, ensure exit criteria are met, and elevate families and children to higher levels of care or refer them to community providers when needed.
