Requirements
- Graduate of an accredited occupational therapy program accredited by ACOTE.
- Hold a current state license as an Occupational Therapist.
- Have at least one year of experience as a licensed Occupational Therapist.
- Have reliable transportation, a valid driver's license, and valid automobile insurance coverage.
- Be willing to travel daily for home visits within the service area.
- Demonstrate strong clinical assessment and treatment-planning abilities.
- Have knowledge of medical terminology and home health regulations.
- Have strong verbal and written communication abilities.
- Be able to work independently with minimal supervision.
- Have organizational skills and attention to detail.
- Be comfortable working in varied home environments.
- Have interpersonal skills for professional interaction with patients, families, and teams.
Responsibilities
- Assess patients' functional abilities and design personalized treatment plans.
- Develop adaptive strategies and solutions to support patients' independence and safety.
- Provide patient care in homes, assisted living facilities, or nursing homes throughout the service area.
- Work one-on-one with patients to rebuild skills for daily activities, including dressing, bathing, cooking, managing routines, and engaging in meaningful activities.
- Work with patients, families, and the interdisciplinary care team to provide education and hands-on training as patients progress.
- Use clinical expertise and problem-solving to help patients regain independence and return to daily activities.
Desired Qualifications
- Home health experience.
- Experience with adaptive equipment and home modifications.
- Knowledge of Medicare home health documentation.
- Strong problem-solving and creative-thinking skills.
