About the job
- A Master's degree in Nursing and graduation from an accredited Nurse Practitioner program are required.
- At least one year of clinical experience as a nurse practitioner is required.
- At least one year of experience in a geriatric specialty is required.
- The ability to assess, diagnose, treat, and follow up is required.
- The ability to work independently and as part of interdisciplinary teams is required.
- The ability to develop relationships and communicate with members and families is required.
- The ability to establish priorities and maintain schedules is required.
- A New York State Nurse Practitioner license is required.
- DEA certificates are required.
- In collaboration with a physician, provide comprehensive assessments, including multifactorial complex physical examinations and psychosocial needs assessments.
- Provide self-management education and tools to help patients better manage multiple conditions.
- Provide and coordinate primary care services for an assigned at-risk panel of Elderplan members residing in the community as part of the Interdisciplinary Co-Management Program.
- Develop care plans and ensure access to preventive care services in accordance with member and Plan health outcome goals and objectives.
- Act as a clinical resource to members, families, and the interdisciplinary team.
- Previous clinical registered nurse experience in an acute, subacute, and/or long-term care setting is preferred.
- Prior experience in managed care is a plus.
