Requirements
  • For the nurse practitioner pathway, graduation from an accredited Master of Science in Nursing or Doctor of Nursing Practice acute care nurse practitioner program for adult care is required.
  • For the physician assistant pathway, graduation from an accredited master's or doctorate degree physician assistant program is required.
  • Nurse practitioners must hold a current, unrestricted registered nurse license recognized by the North Carolina Board of Nursing with authorization to practice as a nurse practitioner.
  • Physician assistants must hold a current, unrestricted medical license issued by the North Carolina Medical Board with authorization to practice as a physician assistant.
  • Both pathways require federal and state Drug Enforcement Administration registration with prescriptive authority.
  • Nurse practitioners must hold Acute Care Nurse Practitioner or Adult-Gerontology Acute Care Nurse Practitioner certification.
  • Physician assistants must hold Physician Assistant-Certified certification.
  • The role requires the ability to practice within the applicable scope of practice in North Carolina and the Duke University Health System.
  • The role requires the ability to use sensory and cognitive functions for health assessments, treatment, and follow-up; fine motor skills; computer skills; and the ability to learn new software programs.
  • The role requires the ability to withstand prolonged standing and walking and to move or lift at least 50 pounds.
Responsibilities
  • Provide and coordinate clinical care for cardiothoracic surgery patients in the Cardiothoracic Intensive Care setting under physician supervision.
  • Round daily with the cardiothoracic surgical, cardiac anesthesiology, intensive care, and medical director teams to assess patients, diagnose conditions, plan treatment, set outcomes, and evaluate care effectiveness and cost efficiency.
  • Perform indicated procedures, including airway management, flexible bronchoscopy, ventilator management, tracheostomy tube changes, chest tube procedures, paracentesis, thoracentesis, vascular access, arterial line procedures, feeding tube placement, catheter management, wound management, electrocardiogram interpretation, defibrillation, cardioversion, temporary pacing, extracorporeal membrane oxygenation cannulation assistance, and mechanical circulatory support device management.
  • Manage acute cardiac or respiratory decompensation, including rapid responses and codes.
  • Collaborate with residents and fellows to formulate treatment plans and monitor patient progress.
  • Assess changes in patient status and initiate emergency interventions until physician support arrives.
  • Conduct preoperative evaluations and establish diagnoses and baseline status.
  • Provide postoperative stabilization and management.
  • Develop patient care pathways, protocols, and guidelines with physicians and other healthcare team members.
  • Analyze clinical practice patterns to improve the effectiveness and efficiency of medical care.
  • Communicate pertinent patient information to supervising physicians, nursing staff, patients, families, and other healthcare providers.
  • Document patient data, diagnostic and laboratory results, patient condition, treatment responses, and communications in the medical chart.
  • Use prescriptive privileges for the identified patient population.
  • Demonstrate competence in nurse practitioner or physician assistant advanced clinical skills.
Desired Qualifications
  • One year of high-acuity adult Cardiothoracic Intensive Care Unit or intensive care unit experience is strongly preferred for nurse practitioners.
  • One year of physician assistant experience in adult high-acuity critical care or Cardiothoracic Intensive Care Unit practice, or completion of a one-year critical care fellowship, is preferred.
