Requirements
  • An associate degree in nursing, social work, or a health-related field is required.
  • A current Basic Life Support certification from the American Heart Association is required.
  • A current Arizona driver's license is required.
  • A fingerprint clearance card application number is required upon hire.
  • A fingerprint clearance card is required within 90 days of hire.
  • At least three years of child development experience is required.
  • Previous computer and word-processing experience is required.
  • Colleagues must understand computers and demonstrate competence using computers and basic software programs.
Responsibilities
  • Provide home-based parenting information and education, developmental assessments, parenting assessments, plan development, emotional support, and preservation of the medical home for at-risk families.
  • Provide referrals and resources based on family needs.
  • Assist supervision with monthly billing documentation and data collection.
  • Perform clerical duties, including collection and data entry for the home visiting program.
  • Follow up on referrals and identified issues.
  • Serve as a community liaison by building, working with, and establishing relationships.
  • Assist with development of community support and education groups.
  • Strengthen parent-child relationships by improving parenting skills, reducing family stress, and meeting the biopsychosocial needs of infants, children, and their parents.
  • Coordinate play groups.
  • Perform initial assessments and evaluations of potential eligible families.
  • Coordinate and administer daily referrals and screenings of patients for admittance into the program.
  • Administer the Parent Survey, or Family Stress Checklist, with families who volunteer to participate in the needs assessment process.
  • Collaborate with the Program Manager or Supervisor to discuss and review each referral.
  • Assist the Supervisor with client satisfaction phone calls and complete appropriate documents.
  • Assist the Supervisor with home observations for staff and complete related documents.
  • Create a safe, trusting atmosphere with families.
  • Ensure that the appropriate curriculum is based on individual family needs.
  • Maintain positive community relationships.
  • Develop and maintain family plans of care, including family progression and accomplishments.
  • Assist the Supervisor with staff development.
  • Assist the Supervisor in orienting new staff.
  • Assist the Supervisor with scheduling educational opportunities for staff.
  • Report safety-related incidents promptly through the Midas/RDE tool.
  • Attend safety-related training programs and perform work safely.
  • Monitor the work environment for possible safety issues and ensure others also work safely.
  • Comply with state and federal regulations, statutes, and company policies affecting the area of responsibility.
  • Keep required certifications and licenses current and valid.
  • Complete mandatory company modules and job-specific training within the specified time frame.
