Requirements
  • The candidate must be at least 18 years of age.
  • The candidate must have a valid driver's license.
  • The candidate must have reliable transportation and provide proof of automobile insurance with $100,000/$300,000/$50,000 liability coverage.
  • The candidate must have a minimum education of a High School Diploma or GED equivalent.
  • The candidate must be able to work in the United States without sponsorship.
  • The role may require lifting up to 35 pounds; lifting more than 35 pounds requires a two-person lift, mechanical lift, or assistive equipment.
  • The role may require prolonged sitting and computer work for up to eight hours.
  • The role may require frequent bending, stooping, and stretching.
  • The role may require prolonged standing for up to eight hours.
  • The employee must wear hearing and eye protection in required areas.
  • The employee must be able to see distant objects and people and perform close visual work involving typed or handwritten materials.
  • The employee must be able to hear conversations in quiet and noisy environments, determine the direction of sounds, and distinguish among bells, buzzers, beeps, and horns.
  • The employee must be able to communicate through speech and remain awake during working hours when applicable.
Responsibilities
  • Hire, schedule, and supervise Direct Support staff; manage training, evaluations, meetings, and documentation; and ensure adequate coverage, including on-call coverage and filling in as needed.
  • Ensure all services follow the Home and Community-Based Services philosophy, agency policies, Department of Human Services regulations, and compliance standards; conduct site visits and maintain quality assurance across programs.
  • Develop, implement, and monitor Individual Service Plans with case managers and the interdisciplinary team; ensure assessments, goals, authorizations, and required documentation are current and accurate.
  • Oversee medication administration processes and appointment follow-ups.
  • Manage individual financial operations, including budgets, ledgers, and payee responsibilities.
  • Serve as the primary liaison with case managers, families, and stakeholders.
  • Manage incident reports, service documentation, transportation coordination, reports, committees, and general agency support tasks.
