Requirements
  • Licensure as a Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
  • Bachelor's Degree in Nursing from an accredited National League of Nursing program with a current license to practice as a Registered Nurse in the State of Rhode Island is required.
  • Three to five years of clinical or case management experience, preferably in the acute and/or community setting with an emphasis on the populations to be served and utilization management.
  • Demonstrated knowledge and skills necessary to provide patient care and to develop or administer care management services with consideration of stages of human development and cultural patterns.
  • Analytical skills necessary to evaluate patients, programs, and procedures.
  • Demonstrated ability to exercise independent judgment and refer specific complex problems to the Manager of Case Management or designee when clarification of departmental policies and procedures may be required.
  • Interpersonal skills necessary to establish and maintain effective working relationships and coordinate case management activities in the clinical area.
  • Ability to lift and carry up to ten pounds and make hospital rounds through various patient care areas.
  • Visual acuity and finger dexterity to review medical records, navigate automated system screens, and type on a computer terminal keyboard.
  • Ability to drive as required.
Responsibilities
  • Provide coordinated care support to Surgical Services, the Total Joint Center, Case Management, and the clinical managers of respective services across the Brown University Health system.
  • Review patients for potential post-acute needs and conduct interviews before scheduled surgeries.
  • Assist with coordination of care at hospitals throughout Brown University Health.
  • Collaborate with the clinical healthcare team across the patient care continuum, including pre-operative service coordination and post-hospital discharge follow-up.
  • Participate in oversight of the progress of a specific patient population.
  • Collect data and facilitate clinical quality improvement teams to enhance quality and cost effectiveness of patient care and prevent readmissions.
  • Work collaboratively with surgical teams and case management to provide continuity in patient care and improve patient outcomes.
  • Collaborate with surgeons' offices to determine patient needs and home safety.
  • Complete initial patient assessments before scheduled procedures.
  • Partner with medical staff and other healthcare team members, in collaboration with patients and families, to facilitate plans of care for a defined patient population across the continuum of care, including after hospitalization.
  • Build post-hospital referrals and coordinate post-acute services before surgery.
  • Deliver Important Message follow-up notices to Medicare patients according to Centers for Medicare & Medicaid Services regulations.
  • Collaborate with the patient's healthcare team to evaluate and facilitate development and implementation of the discharge planning process.
  • Develop the initial patient discharge plan and review it with the patient, family members, and interdisciplinary team as appropriate.
  • Provide care plan direction for a patient care delivery system that supports managed care strategies and decreases readmission risk.
  • Identify opportunities to improve patient flow and reduce service delays through problem resolution and follow-up.
  • Facilitate patient participation in education programs and post-discharge services as necessary.
  • Cross-train to programs within the Case Management Special Programs Department, including Surgical Services Case Manager, Transfer and Access Case Manager, and Outpatient High Risk Case Manager.
  • Perform other duties as assigned.
Desired Qualifications
  • Certification by a recognized national organization in case management is preferred.
