Requirements
  • Hold a current license and active registration as a Nurse Practitioner or Physician Assistant in New York State.
  • Have a master's degree from an accredited Nurse Practitioner program or complete an accredited Physician Assistant program.
  • Be familiar with electronic medical records.
  • Maintain HIPAA confidentiality standards.
Responsibilities
  • Conduct outpatient follow-up visits for patients with cardiovascular conditions, including coronary artery disease, heart failure, arrhythmias, hypertension, and valvular heart disease.
  • Perform pre-operative cardiovascular evaluations and risk assessments for patients undergoing surgical procedures.
  • Obtain detailed medical histories and perform focused cardiovascular examinations.
  • Order and interpret diagnostic studies, including laboratory tests, electrocardiograms, and cardiac imaging, as appropriate.
  • Develop and implement treatment plans in collaboration with supervising cardiologists.
  • Provide medication management and patient education on cardiovascular disease prevention and management.
  • Coordinate care with primary care providers, surgeons, and other specialists.
  • Document patient encounters and treatment plans accurately in the electronic health record.
  • Participate in quality improvement initiatives and practice efficiency efforts.
