Requirements
  • A high school diploma or GED is required; in some cases, experience may substitute for academic training.
  • Applicants must be at least 21 years of age and have a current, valid United States driver's license maintained for the required period.
  • Applicants must be able to obtain CPR and First Aid certification within two weeks of employment, Medication Administration Plan certification within 150 days, Positive Behavior Support Tier 1 certification within 150 days, and Safety Care certification within 90 days.
  • Applicants must successfully pass CORI, reference, national fingerprinting, and driving record checks.
  • Applicants must maintain adequate insurance and a safe driving record when using a personal vehicle.
  • Candidates must be able to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper techniques or a two-person lift, and assist clients with activities of daily living.
  • Candidates must be able to operate a computer and office equipment, communicate with clients and staff, and perform required visual and mobility tasks.
  • Candidates should have strong time-management and writing skills.
  • Candidates should be able to work independently and as part of an interdisciplinary team, communicate using different styles, maintain confidentiality, and apply advocacy, empowerment, participatory planning, household management, crisis intervention, and physical personal-management techniques.
Responsibilities
  • Assist individuals in becoming integrated into their communities and developing and maintaining friendships.
  • Help individuals access community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Support individuals in making choices, self-advocating, and participating in decisions about their supports and lives.
  • Assist individuals with assistive technology and adaptive equipment to improve communication, community access, and independence.
  • Support individuals in securing, maintaining, and developing work, volunteer, and other community-engagement opportunities.
  • Provide training and support, including rehabilitative and behavioral interventions, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning, cooking, household chores, and use of community resources.
  • Perform hands-on program tasks to implement support plans.
  • Provide transportation as necessary.
  • Assess individuals' physical and emotional status and report changes to a supervisor.
  • Monitor individual funds and maintain individual records, progress notes, communication logs, incident records, program records, and routine activity documentation.
  • Procure, store, administer, document, and dispose of medication in accordance with Medication Administration Plan requirements, funding-source regulations, and company policy.
  • Communicate with program nurses, case managers, visiting nurses, families, and other interested parties.
  • Participate in mandated training and staff meetings.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Maintain home safety and perform housekeeping, snow shoveling, trash removal, meal preparation, vacuuming, and grocery shopping as required.
  • Complete communication logs, community membership logs, activity schedules, and other paperwork.
  • Provide shift coverage in other homes or programs when assigned.
  • Function as a human-rights advocate, provide physical intervention or First Aid during behavioral or medical emergencies, and report to the Disabled Persons Protection Commission.
  • Assist in developing, implementing, and documenting treatment plans, Individual Support Plans, Person-Specific Treatment Plans, and clinical programs.
  • Perform other related duties as required.
Desired Qualifications
  • Experience working with people with developmental disabilities and/or brain injury is strongly preferred.
  • One or more years of experience and/or training in a related function is preferred.
