About the job
- Current Registered Nurse license, valid state driver's license, and reliable transportation are required.
- One year of experience as a Registered Nurse in an acute or clinical care setting is preferred.
- Home health experience is preferred.
- Familiarity and comfort with technology are required; Homecare Homebase experience is a plus.
- Evaluate and assess patients to determine their appropriateness for care.
- Use available tools and resources to develop patient-specific care plans with the appropriate level and frequency of care.
- Communicate with the care team, physicians, payors, patients, and families to address care needs and fulfill the patients' care plans.
- Deliver high-quality skilled care and supervise the care of other care-team members.
- Thoroughly document care delivery daily in the electronic medical record system.
- Compassion in care delivery and focus on results.
- Solution-driven, self-motivated approach, with responsiveness and urgency.
- Interest in learning, motivating, and inspiring patients to reach their goals.
- Enthusiasm for working to the highest level of the Registered Nurse license.
