Requirements
  • An active Licensed Practical Nurse license in New York State is required.
  • A high school diploma or equivalent is required.
  • Excellent communication skills are required.
  • The ability to work under supervision, understand the limits of the job description, and learn required skills is required.
Responsibilities
  • Assume the team leader role and accept responsibility and accountability for all resident care on the designated team, including direct physical nursing care.
  • Perform resident care according to policies, procedures, and standards as delegated by the Nurse Manager, Clinical Coordinator, Registered Nurse, or Charge Nurse.
  • Demonstrate appropriate knowledge when performing resident care.
  • Provide emotional and physical comfort and safety for residents, families, and caregivers.
  • Observe residents for specific signs and symptoms as instructed by the Nurse Manager, Clinical Coordinator, or Registered Nurse.
  • Observe changes in clinical or functional status and report them promptly to the Nurse Manager, Clinical Coordinator, or Supervisor.
  • Assist residents with activities of daily living and special care needs for which the nurse is properly educated and trained.
  • Recognize and respond efficiently and effectively to emergent situations by appropriately managing self, residents, staff, and others.
