Requirements
  • An MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required.
  • Must be able to obtain a state medical license or already have a current, active state medical license for the state or states in which the physician will work.
  • Board eligibility is required.
  • Must maintain board certification in the specialty after certification by completing required maintenance of certification, continuing medical education, or board examinations.
  • Must have a current Drug Enforcement Administration number for Schedule II through V controlled substances.
  • Basic Life Support certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
Responsibilities
  • Provide direct primary care to patients and perform assessments in ambulatory health care centers, and occasionally in acute care, nursing homes, skilled nursing facilities, or home settings depending on the assignment.
  • Perform geriatric assessments, obtain medical histories, conduct physical examinations, diagnose and treat conditions, develop plans of care, provide health education, make specialty and case management referrals, conduct follow-up, and document care according to quality and productivity standards.
  • Function independently as a primary care practitioner and assess acute and non-acute clinical problems.
  • Perform and document physical assessments and patient histories, analyze trends in patient conditions, and develop, document, and implement patient management plans.
  • Plan patient care based on the patient population or protocol while considering the patient’s cultural background, level of understanding, personality, and support systems, and serve as a patient advocate.
  • Write admission, transfer, and discharge orders; order and interpret laboratory and diagnostic studies; order medications and treatments; refer patients for consultation; and complete in-depth progress notes and summaries.
  • Participate in patient care rounds and conferences and communicate patient management strategies to the patient care team.
  • Collaborate with multidisciplinary team members to ensure patient management strategies meet patient care needs.
  • Recognize situations requiring immediate attention and initiate life-saving procedures when necessary.
  • Use advanced communication skills to solve complex situations and improve processes and patient services.
  • Analyze and evaluate health care delivery systems with multidisciplinary team members and implement new practice patterns when appropriate.
  • Participate in activities that enhance personal and professional growth and development.
  • Initiate discharge arrangements, write discharge orders, and complete appropriate paperwork.
  • Collaborate with physicians, nurses, physical therapists, social workers, families, and caregivers to transition patients to a lower level of care when medically appropriate.
  • Advocate for patients’ needed services and provide patient and family education.
  • Introduce the primary care provider role to patients and families.
  • Facilitate patient and family conferences to review treatment goals, optimize resource utilization, provide family education, and identify needs.
  • Build collaborative relationships that help patients and families make informed decisions about goals of care, palliative care, and hospice.
  • Manage resource utilization and reimbursement for services.
  • Facilitate discharge to the appropriate level of care and use preferred providers when additional services are required.
  • Critically analyze data and evidence to improve clinical practice.
  • Translate research and other knowledge into improved practice processes and outcomes.
  • Develop new practice approaches based on research, theory, and practice knowledge.
  • Guide change and foster collaboration among patients, communities, integrated health care teams, and policymakers.
  • Advocate for improved access, quality, and cost-effective health care.
  • Develop and implement innovations that advance practice.
  • Continuously improve clinical quality using the best available evidence.
  • Evaluate relationships among quality, safety, access, and cost in health care.
  • Apply peer-review skills to promote a culture of excellence.
