Requirements
  • A bachelor's degree in social work, special education, psychology, nursing, human services, or another field related to the education or treatment of people with developmental disabilities or related conditions.
  • At least one year of case management experience.
  • At least two years of experience working with individuals with disabilities.
  • Knowledge, education, and experience working with individuals with developmental disabilities.
  • Knowledge of local community resources, activities, and social services.
  • Ability to work independently, handle flexibility, and manage stress.
  • Ability to pass a criminal background check.
  • A current driver's license in good standing and reliable transportation.
  • Comprehensive auto insurance.
  • Ability to function independently and relate to a variety of individuals.
  • Professional communication skills.
  • Effective decision-making skills.
  • Ability to create professional reports.
  • Ability to handle crisis situations and follow supervisors' directives.
  • Ability to identify client needs and treatment-plan goals.

Responsibilities
  • Provide case management services to adults accessing waiver and state plan services and other medical, social, educational, vocational, recreational, and related services.
  • Meet with each individual on the caseload.
  • Coordinate services to ensure individuals' wants and needs are met.
  • Maintain confidentiality of records and interactions.
  • Participate in professional meetings, workshops, supervision meetings, and team-based service decisions.
  • Inspect and maintain files so they remain current and contain required documentation.
  • Complete intake and continued-authorization paperwork on time.
  • Complete accurate case notes for each visit and contact.
  • Maintain reliable transportation and provide services in homes, schools, and other community locations.
  • Maintain professional relationships with families and community partners.
  • Develop and maintain positive working relationships within the community.
  • Refer clients to other professionals with supervisor approval.
  • Communicate with other providers involved in clients' services.
  • Maintain knowledge of community resources, agency programs, referral sources, and agencies serving people with cognitive disabilities or mental illness.
  • Identify appropriate services based on individual and family needs.
  • Assess changing individual needs, develop service plans with other providers, link individuals to community resources, and monitor service effectiveness.
  • Participate in other assigned agency functions.

Reach for Resources has been named one of the Top 200 Workplaces in Minnesota by the Minnesota Star Tribune, marking the fourth time the organization has received this honor. This year, Reach for Resources ranked #28 among the Top 200 small organizations. Produced by the same team behind the long-running Star Tribune 100 report of Minnesota's best-performing public companies, the Top Workplaces award recognizes organizations that are leading the way in employee engagement, organizational health, and workplace satisfaction. This year's recognition is based entirely on employee feedback, with 59 Reach for Resources employees participating in the survey.
