A formal diagnosis is not required, but the involvement must be significant enough to relate meaningfully to the experiences of families of people served. Family Connectors must share personal experiences with staff, people served, and members of their chosen family.
  • Must have experience navigating child or adult family-serving systems and teaching family members involved with those systems.
  • Must have a bachelor's degree in a human services field from an accredited school and one year of experience working with children, adolescents, transition-age youth, or adults; or an associate's degree in a human services field from an accredited school and one year of such experience; or a high school diploma or GED and two years of such experience.

  • Must be able to stand, walk, bend, kneel, stoop, crouch, crawl, and climb; lift at least 25 pounds using proper techniques or a two-person lift; operate a computer and office equipment; sit, reach, climb stairs, maneuver through narrow spaces or hallways, and assist persons served with daily-living tasks. Must be able to remain stationary 50% of the time as needed, move around the facility, communicate with persons served, staff, and external representatives, and meet the specified close, distance, color, peripheral, and depth vision and focus-adjustment abilities.
  • CPR certification is required within the first two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • MAP certification is required within 150 days of hire.
Responsibilities
  • Create a welcoming environment for parents, families, and community supporters, orient them to ACCS services, share personal experiences, engage their participation in helping persons served meet ACCS goals, and serve as the primary liaison for families following authorization under HIPAA.
  • Conduct outreach and collect current and historical information about families and supporters, focusing on relationships with persons served and their impact.
  • Provide family psychoeducation to families, supporters, and guardians, and refer them to community family-support and education resources, including NAMI Family-to-Family groups, when appropriate.
  • Collaborate with persons served and their families or supporters to determine how they can support the desired level of family participation in treatment, addressing situations where supporters are welcomed or not welcomed, or where persons served want participation that is not occurring.
  • Train and mentor staff in maintaining a family/supporter perspective in their work.
  • Participate on intervention teams for assigned persons served and, with supervision from clinically licensed team members, deliver Treatment Plan interventions that build skills in daily living, medication administration, symptom reduction, health and wellbeing, employment, education, socialization, and recreation.
  • Use trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based approaches and skills-training techniques.
  • Provide competency-based clinical, evidence-based, and best-practice guidance to team members and GLE/SIE staff, including Motivational Interviewing; Harm Reduction and Addiction Treatment; Screening, Brief Intervention, Referral and Treatment; Housing First; Stages of Change; Seeking Safety; Wellness Recovery Action Plan; Cognitive Behavioral interventions; Safety Plans; Crisis Prevention and Response Plans; and the Eight Dimensions of Wellness.
  • Deliver evidence-based clinical and recovery-oriented interventions to persons served, including Motivational Interviewing; Harm Reduction and Addiction Treatment support; Screening, Brief Intervention, Referral and Treatment; Housing First; Stages of Change; Wellness Recovery Action Plans; Seeking Safety; and Cognitive Behavioral interventions.
  • Promote access to healthcare, employment, and education services for persons served.
  • Promote engagement, housing permanency, community tenure, greater self-sufficiency, and successful completion of services for persons served.
  • Document the needs and services of persons served thoroughly and thoughtfully.
  • Create and sustain the ACCS Family Advisory Council if families or supporters express interest in joining.
  • Participate in the Family Connector Community of Practice Workgroup for ongoing role-specific competency development, mentoring, and learning.
  • Perform other related duties as assigned.
Desired Qualifications
  • Bachelor's degree from an accredited school or an associate's degree is strongly preferred.
  • Bilingual/bicultural applicants and applicants with lived experience of psychiatric conditions are preferred.
