- Three to five years of experience in an advanced life support environment as a paramedic.
- A high school diploma or GED is required.
- A bachelor's degree in Healthcare or a Public Health-related field is required; the degree requirement may be waived with 10 or more years of paramedic experience.
- NJ Mobile Intensive Care Paramedic or Mobile Intensive Care Nurse licensure is required, or the candidate must be eligible for New Jersey reciprocity.
- Community Paramedicine Certification is required or must be obtained within one year of hire.
- Basic Life Support, Advanced Cardiac Life Support, Pediatric Advanced Life Support, Emergency Pediatric Care, or Pediatric Education for Prehospital Professionals certification is required upon hire.
- The ability to communicate with patients, visitors, and coworkers is required.
- Conduct comprehensive in-home assessments to identify medical, behavioral, and social needs in patients' home environments.
- Communicate and coordinate with healthcare providers by relaying findings to primary care, care managers, and specialty teams to help close care gaps.
- Coordinate access to community-based health services, including primary care, public health, community-based programs, disease management, prevention and wellness, and mental health services.
- Identify changes in patient status requiring immediate initiation of 911 emergency services or communication with transitional care members and other medical providers to determine treatment plans.
- Provide clinical care within the New Jersey paramedic scope of practice in accordance with applicable New Jersey regulations and agency-specific clinical protocols.
- Serve as part of a multidisciplinary team and support people with substance use disorder and their families in accessing care or support.
- Provide follow-up, in partnership with a peer recovery coach or patient navigator and as part of a multidisciplinary team, to patients recently reversed with Naloxone or administered an opioid partial agonist-antagonist.
- Support chronic disease management by monitoring and reinforcing care plans for conditions such as congestive heart failure, chronic obstructive pulmonary disease, diabetes, hypertension, and substance use disorder.
- Provide post-discharge home visits to reduce morbidity and hospital admissions or readmissions.
- Educate first responders about resources available to patients with substance use disorder so they can better provide this information to patients and families.
- Bilingual English and Spanish skills are preferred.
- National Registry of Emergency Medical Technicians Paramedic certification is preferred.
