Requirements
  • The Crisis Center Licensed Practical Nurse must be licensed to practice as a Licensed Practical Nurse in the State of Washington.
  • The Licensed Practical Nurse must have successfully completed the state-mandated HIV education.
  • The Licensed Practical Nurse must obtain continuing education credits to meet licensure requirements.
  • The position requires Basic Life Support cardiopulmonary resuscitation certification within 90 days of hire and current certification throughout employment.
  • The employee must complete a pre-hire and annual tuberculosis test screening.
  • The employee must complete annual health screening and receive annual influenza and COVID-19 vaccines.
Responsibilities
  • Coordinate closely with clients, providers, and interdisciplinary teams to deliver comprehensive care, including direct clinical services such as medication administration, insulin management, and wound care, and ensure timely implementation of provider-ordered interventions.
  • Conduct thorough assessments and maintain accurate clinical documentation, including monthly summary notes, evaluations, medication records, alert charting, and compliance with documentation and quality-control protocols.
  • Develop, update, and implement individualized care and service plans in response to changes in condition, hospitalizations, or behavioral needs, including crisis interventions, medication refusals, and special precautions.
  • Ensure compliance with clinical, safety, and regulatory requirements by adhering to infection-control practices, emergency protocols, medication-management standards, and reporting obligations concerning abuse, neglect, and significant condition changes.
  • Maintain flexibility to meet staffing and operational needs.

  • Mental health experience is preferred.
