Requirements

Responsibilities
  • Conduct comprehensive assessments of members' health status.
  • Develop and implement individualized care plans.
  • Observe, identify, and document signs and symptoms of changes in the client's physical, emotional, mental, or social condition, reactions and medications, and environmental safety factors.
  • Administer medications and treatments as prescribed.
  • Maintain accurate and timely records and preserve the confidentiality of medical records.
  • Educate members and their families on proper healthcare procedures and disease management.
  • Collaborate with other healthcare professionals to ensure coordinated and effective care.
