RN Care Manager - Home Health and Private Pay
About the job
- An active California Registered Nurse license is required.
- At least 2–3 years of home health nursing experience is required.
- Experience managing private-pay clients is required.
- Strong computer skills are required.
- The candidate must be able to multitask, drive, and take action.
- Strong organizational and communication skills are required.
- The candidate must pass Live Scan fingerprint clearance.
- A valid driver's license and proof of insurance are required.
- A cell phone with internet access is required.
- Proof of negative tuberculosis test results is required.
- The candidate must be willing to work one weekend a month as backup on-call.
- Serve as the primary care manager and coordinate client care among caregivers, registered nurses, therapists, and outside representatives.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Provide ongoing feedback to clients, client representatives, caregivers, and nurses.
- Work closely with the office team.
- Provide high-quality care to older adults in their homes.
- A master's degree in Nursing is preferred.
- Industry experience is preferred.
