- Current registration as a Licensed Practical Nurse in the applicable province or state.
- Strong clinical knowledge and care coordination skills.
- Excellent communication, critical thinking, and time management abilities.
- Current cardiopulmonary resuscitation and First Aid certification.
- Proficiency with electronic documentation and Microsoft Office.
- Completion of a current Vulnerable Sector Check, or willingness to obtain one.
- Work collaboratively with the care team using a person-centered care approach.
- Conduct and maintain accurate resident assessments, care plans, and documentation, and communicate updates as needed.
- Implement and evaluate care plans, adjusting them as required.
- Participate in resident care conferences and support scheduling.
- Assist with resident transitions, including move-ins, discharges, and transfers.
- Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
- Safely manage medications and narcotics in accordance with scope of practice and legislation.
- Educate residents and families about health conditions and care plans.
- Coordinate care plans with physicians, team members, and service providers.
- Assign responsibilities to the nursing team for efficient service delivery.
- Respond to significant changes in residents’ conditions and manage medical emergencies.
- Provide direction and delegate tasks to unregulated care providers.
- Oversee emergency response in the absence of senior leadership.
- Address resident and family concerns and ensure timely follow-up or escalation.
- Support infection prevention, health and safety initiatives, and quality improvement plans.
- Act with integrity and uphold the organization's values, particularly those related to ethics and trust.
- Perform other related duties as assigned.
- Experience in senior care or long-term care.
- Education or background in gerontology, rehabilitation, or complex or community care.
