Requirements
  • Current registration as a Licensed Practical Nurse in the applicable province or state.
  • Strong clinical knowledge and care coordination skills.
  • Excellent communication, critical thinking, and time management abilities.
  • Current cardiopulmonary resuscitation and First Aid certification.
  • Proficiency with electronic documentation and Microsoft Office.
  • Completion of a current Vulnerable Sector Check, or willingness to obtain one.
Responsibilities
  • Work collaboratively with the care team using a person-centered care approach.
  • Conduct and maintain accurate resident assessments, care plans, and documentation, and communicate updates as needed.
  • Implement and evaluate care plans, adjusting them as required.
  • Participate in resident care conferences and support scheduling.
  • Assist with resident transitions, including move-ins, discharges, and transfers.
  • Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
  • Safely manage medications and narcotics in accordance with scope of practice and legislation.
  • Educate residents and families about health conditions and care plans.
  • Coordinate care plans with physicians, team members, and service providers.
  • Assign responsibilities to the nursing team for efficient service delivery.
  • Respond to significant changes in residents’ conditions and manage medical emergencies.
  • Provide direction and delegate tasks to unregulated care providers.
  • Oversee emergency response in the absence of senior leadership.
  • Address resident and family concerns and ensure timely follow-up or escalation.
  • Support infection prevention, health and safety initiatives, and quality improvement plans.
  • Act with integrity and uphold the organization's values, particularly those related to ethics and trust.
  • Perform other related duties as assigned.

  • Experience in senior care or long-term care.
  • Education or background in gerontology, rehabilitation, or complex or community care.
