- Establish close relationships with patients and serve as their point of contact during initial onboarding and orientation, with monthly engagements to help them navigate their NorthStar Care treatment.
- Assist patients with registering for patient portal sites and facilitate consent forms for treatment.
- Coordinate medical appointments with the Care Team.
- Communicate with Care Team members, including primary care physicians, nursing staff, peer guides, and other healthcare providers, to facilitate patient care.
- Order patient treatment supplies for home delivery.
- Identify resources to help patients overcome barriers to treatment.
- Stay aware of current services and education offered by NorthStar Care providers.
- Maintain strict confidentiality in accordance with NorthStar Care policies.
- Meet with the Care Team, including the Care Coordinator and primary care provider, to discuss client care issues and needs during treatment.
- Document all patient encounters and complete reporting requirements according to organization standards.
- Confidentially track patient information, schedules, files, faxes, and forms.
- Track patient attendance at medical appointments and navigation sessions, and initiate outreach and missed-appointment procedures as necessary.
- Bilingual ability in English and Spanish is preferred but not required.
- Computer literacy.
