Requirements
  • Must be a graduate of an accredited School of Nursing.
  • Must hold a Registered Nurse license in the Commonwealth of Massachusetts.
  • Must possess a current CPR certificate.
  • Previous outpatient and/or acute-care experience is desirable.
  • Must have interpersonal and communication skills.
  • Must be able to work independently and be well-organized and priority focused.
Responsibilities
  • Assess new patients for healthcare needs and link each patient to the provider that best meets their assessed needs.
  • Coordinate patient access issues so patients receive needed care.
  • Obtain and make available relevant patient information and medical records before provider appointments.
  • Coordinate patient calls seeking access to care, treatment, and services through the call center.
  • Develop and implement a follow-up system for patients who use the Emergency Room, including education about appropriate use of emergency and ambulatory care services.
  • Supervise Access medical assistants and community health workers who support access duties, Emergency Room follow-up, and case coordination.
  • Facilitate patient care by assessing needs and helping patients obtain education and resources to manage chronic illness, acute episodes, and exacerbations.
  • Use nursing skills, knowledge, and the nursing process to assist patients and families with self-care, compliance with healthcare recommendations, and optimal health.
  • Obtain documentation related to hospitalizations, Emergency Room visits, and other medical and behavioral records as part of care coordination.
  • Assess every new patient, including medical history, allergies, psychosocial problems, medications, and current living arrangements of patients and their families.
  • Provide age-appropriate care based on the knowledge and skills required for the patients served.
  • Assist with established-patient care coordination activities related to Medicaid managed-care organization shared savings, meaningful-use payments, and PHO activities.
  • Participate in a multidisciplinary approach directed toward client needs and care.
  • Communicate patient health-status information with healthcare providers, including providers, social workers, and nutritionists, and involve family members or significant others in the plan of care as appropriate.
  • Document care based on assessment data, nursing knowledge, and the available referral network.
  • Educate patients and families about Lowell Community Health Center and its services in a clear and culturally, age, and language-sensitive manner; document teaching materials, responses, comprehension, and follow-up needs.
  • Demonstrate clinical competence and professional commitment to nursing practice by adhering to standards of practice and using appropriate scientific principles.
  • Attend staff and community meetings and participate in community education and health-promotion programs when requested.
  • Communicate regularly with nurses, nurse practitioners, and physicians for professional information, support, and quality assurance of nursing services.
  • Participate in orientation and teaching processes for new employees and students when requested.
  • Provide patient care when needed and provide patient education and follow-up.
  • Apply principles of teaching and learning, growth and development across the lifespan, and family dynamics.
  • Assess patient-health-status data and interpret it in the context of each patient's specific needs.
  • Perform additional duties and responsibilities consistent with the role.

Lowell Community Health Center appoints Tom Murphy as Chief Financial Officer. February 11, 2026 Lowell Community Health Center today announced the appointment of Tom Murphy as Chief Financial Officer, following his service as Interim CFO since July 2025. Murphy brings extensive financial and operational experience to the role, supporting the organization's ongoing work and priorities. During his time as Interim CFO, he worked closely with the finance and executive teams, providing steady leadership and oversight across the health center's financial systems. Prior to serving as Interim CFO, Murphy was Senior Director of Finance at Lowell Community Health Center. His collaborative leadership style and ability to manage complex financial work while keeping teams informed and supported have played an important role in supporting the organization's ongoing operations. "Tom stepped into the Interim CFO role with a clear understanding of our financial operations," said Susan West Levine, Chief Executive Officer of Lowell Community Health Center.
