Requirements
  • Current nursing license to practice in the state where the employee provides service.
  • Current driver's license in the state of residence and possession of a registered and insured motor vehicle.
  • Two years of experience working in an acute care setting.
  • Graduation from an accredited school of nursing.
Responsibilities
  • Respond to and visit patients referred for admission to Calvary @ Home.
  • Inform patients, caregivers, and family members about available hospice services and assess patients for admission.
  • Visit Calvary @ Home patients who have been admitted to the hospital and communicate patient status with the respective interdisciplinary team.
  • Develop relationships with hospitals, physicians, nursing facilities, and other referral sources through outreach, education, and networking.
  • Educate patients and caregivers about hospice, evaluate eligibility, and work with the interdisciplinary team to develop an initial plan of care using hospice and palliative care principles.
  • Generate and respond to referrals from hospitals and other community partners.
  • Work with physicians and the interdisciplinary team.
  • Travel on weekends.
Desired Qualifications
  • One year of hospice experience.
  • Bachelor of Science in Nursing preferred.
