Requirements
  • A minimum of 3 years of experience working with adult and geriatric populations as an Advanced Practice Provider, Nurse Practitioner, or Physician Assistant.
  • Current knowledge and understanding of primary care, chronic care management, and best practices.
  • A Master's Degree as a Nurse Practitioner or Physician Assistant.
  • American Heart Association Basic Life Support certification.
  • New Jersey Advanced Practice Nurse and New Jersey Registered Nurse license, or New Jersey Physician Assistant license.
  • A valid state driver's license with a clean Department of Motor Vehicles record.
  • A copy of recent vehicle insurance.
  • Evidence of continuing professional development and maintenance of clinical skills.
  • Completion of required continuing education mandated by certifying organizations.
Responsibilities
  • Provide in-home longitudinal primary care for adult patients.
  • Practice autonomously and/or in consultation with collaborating or supervising physicians and other healthcare professionals.
  • Assess, diagnose, treat, and manage patients' health problems and needs.
  • Perform patient histories and physical examinations to determine working differential diagnoses and develop individualized plans of care.
  • Order, perform, supervise, and interpret diagnostic and laboratory tests.
  • Prescribe pharmacologic agents and non-pharmacologic therapies in accordance with applicable guidelines and state and federal law.
  • Provide therapeutic measures and deliver teaching, education, and counseling to individuals, families, and groups.
  • Formulate diagnoses for uncommon or unstable conditions in consultation with collaborating or supervising physicians.
  • Refer patients to specialists or participate in dual management and treatment with collaborating or supervising physicians.
  • Diagnose and manage acute and chronic illnesses while emphasizing health promotion and disease prevention.
  • Serve as a healthcare researcher, professional educator, interdisciplinary consultant, and patient advocate.
  • Document prescribed medications and authorized refills in the patient's chart.
  • Participate in peer review and clinical outcome evaluations.
  • Adhere to the code of ethical practice and provide evidence-based medical care.
  • Maintain accountability for healthcare outcomes and remain current with medical trends.
