Requirements
  • Maintain an active registered nurse license in good standing.
  • Have at least 3 years of clinical practice in an intensive care unit, or in a hospice or palliative care role.
  • Have experience conducting goals-of-care discussions and discussions regarding end-of-life care decisions.
  • Be able to pivot quickly on priorities in a rapidly changing startup environment.
  • Have intermediate skills with Microsoft Office Suite products, including Outlook and Teams.
  • Work effectively in a remote environment with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
  • Work from a dedicated space without ongoing interruptions to protect member protected health information and HIPAA information.
Responsibilities
  • Build empathetic relationships with patients and families.
  • Complete patient education, including family involvement as appropriate, regarding diagnosis, prognosis, and treatment options, and advocate for access to the preferred treatment option.
  • Evaluate and identify patient needs based on their values, goals, and preferences by completing advance care planning and goals-of-care discussions, then translate those needs into clinical needs.
  • Apply critical thinking to analyze collected data.
  • Identify opportunities to improve symptom palliation and collaboratively develop a plan of care with patients’ providers.
  • Engage internal resources to identify and respond to social determinants of health, including transportation, housing, and food-resource needs.
  • Participate in huddles, interdisciplinary team sessions, and patient case conferences as appropriate.
  • Interface with nephrology partners, primary care physicians, advanced practice providers, specialists, and other disciplines to develop case-management plans and programs.
  • Develop plans of care with input from patients, families, and the interdisciplinary team following goals-of-care discussions.
  • Maintain accurate records of care associated with each patient.
  • Communicate and implement plans of care in all settings, including with community palliative-care and hospice providers.
  • Coordinate care and access to health services across multiple providers and disciplines, including home health, palliative care, hospice, pastoral care, and counseling.
  • Evaluate ongoing progress toward appropriate outcomes and care transitions.
  • Reassess and revise plans of care as needed through ongoing goals-of-care discussions, collaboration with patients and families, input from the interdisciplinary team, and appropriate documentation.
  • Maintain current knowledge and competency in hospice and palliative-care nursing.
  • Educate internal and external community partners about the Evergreen Compassionate Care Program and how to identify and refer patients to it.
  • Represent Evergreen to patients, families, and the community.
  • Perform other duties consistent with the role as assigned.
Desired Qualifications
  • Certified Hospice and Palliative Care Nurse certification.
  • Compact registered nurse licensure.
  • Bachelor of Science in Nursing degree.
  • At least 1 year of utilization-management experience.
