Requirements
- Graduate of an accredited occupational therapy program (ACOTE accredited).
- Hold a current state license as an Occupational Therapist.
- Have a minimum of 1 year of experience as a licensed Occupational Therapist.
- Have reliable transportation, a valid driver’s license, and valid automobile insurance coverage.
- Be willing to travel daily for home visits within the service area.
- Demonstrate strong clinical assessment and treatment planning abilities.
- Have knowledge of medical terminology and home health regulations.
- Demonstrate problem-solving and creative thinking skills.
- Have verbal and written communication abilities.
- Be able to work independently with minimal supervision.
- Have organizational skills and attention to detail.
- Be comfortable working in varied home environments.
- Have interpersonal skills for professional interaction with patients, families, and teams.
Responsibilities
- Assess patients’ functional abilities and design personalized treatment plans.
- Develop adaptive strategies and solutions that support patients’ independence and safety.
- Provide one-on-one care to patients in their homes, assisted living facilities, or nursing homes.
- Guide patients in rebuilding skills for daily activities, including dressing, bathing, cooking, managing routines, and engaging in meaningful activities.
- Work closely with patients, families, and an interdisciplinary care team.
- Provide education, hands-on training, and encouragement as patients work toward greater confidence and independence in daily life.
Desired Qualifications
- Home health experience.
- Experience with adaptive equipment and home modifications.
- Knowledge of Medicare home health documentation.
