Requirements
  • At least 3 years of patient-facing experience.
  • Completion of an accredited nurse practitioner or physician assistant program with national certification.
  • An unrestricted nurse practitioner or physician assistant license to practice in the state of employment with Cityblock.
  • Board certification or board eligibility within one year of employment.
  • Certification in Basic Life Support for Healthcare Providers.
  • DEA licensure or willingness to acquire it.
  • Completion of Cityblock’s medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, and Varicella, and a tuberculosis screen, unless an approved medical or religious accommodation applies.
Responsibilities
  • Conduct comprehensive clinical intake visits and provide longitudinal direct primary care or wrap-around care focused on chronic condition management and preventative care.
  • Provide timely follow-up for members after a transition-of-care event, an acute need, or according to condition-specific needs.
  • Proactively reach out to members who need follow-up care.
  • Complete comprehensive and compliant diagnosis capture related to Hierarchical Condition Categories and International Classification of Diseases-10 coding.
  • Address contractual and company-prioritized quality gaps and ensure proper chart documentation and International Classification of Diseases or Current Procedural Terminology codes are included in encounters as supporting evidence of gap closure.
  • Prescribe first- and second-line medications for depression, anxiety, alcohol use disorders, and opioid use disorders.
  • Remain current on evidence-based care for complex adult primary care patients, with pediatric care in certain markets.
  • Refer members to specialist care appropriately and according to internal CBH guidance when available.
  • Support members in achieving physical, behavioral, and social health goals in their care plans.
  • Participate in case conferences with the care team and provide clinical expertise on care plan development.
  • Provide guidance on high-impact clinical interventions and tasks to achieve members’ goals.
  • Collaborate on a panel of members to provide clinical input, including health maintenance, chronic disease management, and support for co-occurring psychiatric disorders.
  • Support members through scheduled appointments, walk-ins, and urgent or emergent visits across face-to-face and virtual modalities and in hub, community, or home settings.
  • Assist with the development and enhancement of clinical programs, initiatives, processes, and policies.

Homeward Health, founded in 2022 by former Livongo executives, has delivered care to more than 50,000 rural patients across 50 counties. The acquisition pairs Cityblock's data platform with Homeward's rural provider relationships. Cityblock launched in 2017 from Alphabet's Sidewalk Labs, focusing on integrated health and social services for marginalized communities. The company operates in over 10 states, partnering with Medicaid plans and health systems.
