Requirements
  • An active Colorado Speech-Language Pathologist license in good standing is required.
  • Strong clinical reasoning and the ability to operate independently in the field are required.
  • The clinician must be able to identify red flags and escalate changes in condition to a registered nurse or physician.
  • Caregiver education skills, including teach-back and carryover planning, are required.
  • Strong communication and documentation skills are required.
  • Reliable follow-through on orders, frequencies, and patient goals is required.
  • A current driver's license and comfort with daily travel across the assigned territory are required.
Responsibilities
  • Own the patient's Speech-Language Pathology plan, progression, and functional outcomes.
  • Complete comprehensive in-home Speech-Language Pathology evaluations and deliver skilled visits.
  • Assess and treat aphasia and other communication disorders as ordered or clinically indicated.
  • Assess and treat cognitive-communication impairments involving attention, memory, and executive function that affect safety and independence.
  • Conduct dysphagia screening and assessment and provide swallow-safety education within home-health scope and orders.
  • Build and execute a plan of care with measurable, functional goals.
  • Coordinate with the multidisciplinary care team, communicate changes in condition, align on orders, and keep care moving.
  • Coach patients and caregivers on practical strategies and routines.
  • Document promptly and thoroughly in a defensible, efficient, and consistent manner.
Desired Qualifications
  • Prior home-health experience is preferred.
  • Experience with Homecare Homebase or a similar home-health electronic medical record is preferred.
  • Neuro experience involving stroke, aphasia, or cognitive impairment is preferred.
  • Dysphagia-related experience appropriate for home health is preferred.
