Requirements
  • A nursing degree from an accredited college or university is required.
  • A current unrestricted Licensed Practical Nurse license in the practicing state is required.
  • The ability to demonstrate compassion when working with residents is required.
Responsibilities
  • Provide direct nursing care for residents and general consultation regarding health-related concerns within the community.
  • Direct the daily activities of certified nursing assistants and resident care associates in accordance with applicable regulations and company policies and procedures.
  • Follow the nursing model to promote holistic care for residents.
  • Ensure that policies and procedures are followed by assigned nursing personnel.
  • Participate in resident admissions, discharges, and transfers as required.
  • Assess changes in residents’ status, notify the physician and resident’s family or representative, and document accordingly.
  • Transcribe physician orders to the medical record and carry out orders as written.
  • Prepare and administer medications according to physicians’ orders and observe for adverse effects.
  • Perform wound treatments according to physicians’ orders and document changes as indicated.
  • Schedule resident follow-up appointments and transportation needs as indicated.
  • Provide nursing leadership to personnel assigned to the unit.
  • Perform direct-care skills including colostomy changes, tube feedings, wound care, suctioning, and intravenous administration in accordance with policies and procedures.
  • Perform rounds to ensure resident needs are met and personnel perform assigned duties.
  • Collaborate with interdisciplinary team members to ensure residents’ needs are holistically met.
  • Report incidents or unusual occurrences to the community’s leadership team.
  • Ensure adequate stock of medications, supplies, and equipment, and notify appropriate personnel of needs.
  • Initiate, review, and update care plans as required.
  • Participate in inspection survey processes when surveys arise.
  • Participate in quality assurance and performance improvement activities as needed or assigned.
  • Ensure infection-control principles are followed and help correct deficiencies in practice.
  • Comply with all policies, procedures, and updates.
  • Perform other assigned duties.
Desired Qualifications
  • At least one year of nursing experience is preferred.
