Requirements
  • A high school diploma or GED/equivalent is required.
  • A current Licensed Practical Nurse license in the state of service is required.
  • A valid United States driver's license is required, along with an acceptable driving record and required vehicle insurance when using a personal vehicle.
  • Applicants must be at least 21 years of age.
  • Applicants must obtain CPR and First Aid certification within two weeks of employment and Medication Administration Program certification within 150 days of employment.
  • Applicants must successfully pass CORI, reference, multi-state background, and driving record checks.
  • Applicants must be able to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, and assist clients with activities of daily living.
  • Applicants must be able to operate a computer and office equipment and communicate with clients, staff, and external representatives.
  • Applicants must comply with accreditation and licensing requirements.
Responsibilities
  • Participate in comprehensive physical-health assessments of people served in a clinically intensive program.
  • Develop and implement person-centered treatment care plans.
  • Provide nursing care and assistance with activities of daily living, including showering, dressing, shaving, mouth care, foot care, diabetes care, wound care, GT/JT care, and nutritional support.
  • Monitor clients for changes in status, perform individualized assessments, and provide skilled nursing interventions as indicated.
  • Administer medications and treatments as ordered by the primary care provider and document them according to policy and procedure.
  • Provide transportation and accompany individuals to medical, dental, and other appointments as needed.
  • Ensure that individuals' nutritional needs are met and that medical, nutritional, pharmacological supplies, and medical equipment are ordered safely and on time.
  • Use rehabilitation values, principles, techniques, skills training, behavioral strategies, and prescribed clinical plans to help individuals overcome barriers and live more independently.
  • Supervise CNAs during designated shifts and ensure that care is timely, accurate, and aligned with individual service plan goals.
  • Provide relevant nursing education and training to CNAs as needed.
  • Communicate changes in client status to the Program Director, primary care provider, family or guardian, and other appropriate agencies.
  • Maintain communication with the Program Nurse Administrator, primary care provider, families or guardians, Vinfen employees, outside agencies, community agencies, and neighbors.
  • Use the emergency and on-call system when appropriate.
  • Complete and maintain shift notes, daily communications, treatment plans, medical records, and incident reports, including medication errors, in compliance with regulatory standards and Vinfen policy.
  • Maintain a safe, clean, comfortable, and welcoming environment that meets regulatory, credentialing, fire, safety, and organizational requirements.
  • Store medications and treatment supplies safely and maintain clients' medical equipment and supplies, including wheelchairs, walkers, Hoyer lifts, and hospital beds.
  • Support individuals' participation in community outings and events while respecting their preferences and choices.
  • Advocate for people served and collaborate with Vinfen departments, outside agencies, and community providers.
  • Attend and actively participate in treatment-plan, individual service plan, and other planning meetings as warranted.
  • Develop and maintain effective relationships with individuals served, families or guardians, neighbors, and other significant people.
  • Participate in Vinfen training, conferences, and educational opportunities as indicated.
  • Perform other assigned duties and responsibilities.
Desired Qualifications
  • A bachelor's degree in Social Work, Psychology, or a related field is preferred, with experience potentially substituting for education.
  • An Associate's degree is preferred.
  • One to two years of supervised practical experience, including internships, in a clinical or rehabilitation setting is preferred or helpful.
  • Experience working with people with psychiatric conditions, developmental disabilities, medical illnesses, or co-occurring disorders is preferred.
  • Preference is given to bilingual or bicultural applicants and applicants with lived experience.
  • Knowledge of Licensed Practical Nurse practice standards and intervention techniques.
  • Knowledge of developmental disabilities, evidence-based practices, assessment practices, documentation standards, teaching and learning modalities, communication and learning styles, supervisory techniques, community health and mental health services, and personal computer applications.
  • Ability to develop supportive relationships, supervise and mentor CNAs, make independent judgments, work confidentially, and use proper body mechanics.
