Requirements
  • Prior experience as a Certified Nursing Assistant is required; experience working with people with brain injuries is preferred.
  • A high school diploma or equivalent is required.
  • A valid driver's license must be presented at employment. The employee must be at least 21, have held a valid United States driver's license for at least six months, maintain adequate insurance if using a personal vehicle, and pass a driver's screening background check.
  • The employee must be able to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper techniques or a two-person lift, and assist people served with activities of daily living. The role also requires operating a computer and office equipment, sitting, reaching, climbing stairs, maneuvering through narrow spaces, communicating, and the specified close, distance, color, peripheral, and depth vision and ability to adjust focus.
  • The employee must obtain CPR and First Aid within two weeks of hire, Medication Administration Program certification within 150 days, and Safety Care certification within 90 days. The employee may be asked to obtain Certified Brain Injury Specialist certification.
  • A Certified Nursing Assistant license in the state of service is required.
  • The role requires skills and competencies of a Certified Nursing Assistant; knowledge of brain injury, human/legal/civil rights and community resources, empowerment and self-advocacy techniques, teaching modalities, personal computer applications and equipment, and documentation standards; ability to communicate verbally and document nursing care accurately; collaborate effectively; balance competing priorities; make independent judgments and decisions; and work professionally and confidentially.
  • The employee must demonstrate sensitivity to cultural, religious, ethnic, disability, and gender issues.
Responsibilities
  • Provide overnight program activities and direct daily and episodic nursing aide care in a community living arrangement for people with brain injuries and complex health needs.
  • Provide assistance with activities of daily living according to each person's treatment plan and individual service plan, including community membership and relationships, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range-of-motion exercises as needed or ordered by the primary care provider.
  • Monitor and observe people served for changes in physical, mental, or emotional status.
  • Administer medication and treatments according to Medication Administration Program guidelines and Vinfen policies and procedures.
  • Provide for the nutritional needs of people served.
  • Train and support people served in completing activities of daily living, including showering, personal hygiene, dressing, and household chores.
  • Conduct fire and safety training for people served and document it as required.
  • Provide transportation as necessary to medical, dental, and other appointments as directed by the shift or program manager or administrator.
  • Maintain a safe, clean, welcoming, and respectful environment in individual rooms and common areas, meeting safety and regulatory standards; perform related housekeeping, snow shoveling, and garbage removal as needed.
  • Document nursing care and services, medication administration, and daily shift notes according to Vinfen policies and procedures; keep medical records, confidential files, and other information current and accurate.
  • Follow incident-reporting and medication-error procedures and ensure paperwork meets Vinfen and outside-agency standards.
  • Communicate effectively and accurately with managers, administrators, colleagues, Vinfen staff, families, and guardians; report changes in a person's status as directed.
  • Use assistive and medical equipment safely, including wheelchairs, Hoyer lifts, walkers, glucometers, nebulizers, and oxygen equipment; maintain individuals' medical equipment and supplies.
  • Provide opportunities for people served to participate in home, program, and community events, while encouraging and respecting their preferences and choices.
  • Advocate effectively for people served and maintain effective relationships with them, their families or guardians, and other significant people in their lives.
  • Liaise and collaborate with Vinfen departments, outside agencies, and community providers.
  • Attend and actively participate in staff training, staff meetings, and community meetings.
  • Act as a positive role model for people served.
  • Assist with orientation of new staff.
  • Adhere to Vinfen policies and procedures and perform other related duties as assigned.
