Requirements
  • Current licensure to practice as a licensed vocational nurse, registered nurse, or master's-prepared social worker in Texas.
  • Three to five years of experience in nursing or medical social work, including experience in the assigned specialty.
  • A valid driver's license and reliable transportation are required because driving between campuses is necessary.
  • The ability to work independently with sound judgment, discretion, initiative, problem-solving ability, and strong organizational skills is required.
  • Knowledge of cost and quality issues related to patient care and the integration and coordination of health services is required.
  • The ability to maintain confidentiality, communicate effectively, type, and use computers at a basic level is required.
Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate options and services needed to meet patients' health care needs through all phases of wellness and sickness.
  • Collaborate with patients, families, payers, and the multidisciplinary healthcare team.
  • Maximize efficient utilization of resources and promote quality, cost-effective outcomes.
Desired Qualifications
  • Prior experience in case management or discharge planning is a plus.
  • LVN, RN, or licensed master's social worker with CCM, CPUR, or ACP certification is preferred.
