Requirements
  • Graduate of an accredited therapy program.
  • Two to three years of related experience, or an equivalent combination of education and experience.
  • Current Occupational Therapist state license and CPR certification.
  • Ability to communicate clearly with patients in person and over the phone.
  • Ability to lift, transfer, and assist patients through physical activities, including frequently lifting and carrying up to 50 pounds unassisted.
  • Visual acuity adequate to examine patients and read printed and computer-based information.
  • Ability to work around blood, body fluids, and infectious drainage while following Universal Precautions.
  • Time management, sound judgment and decision-making, active listening, service orientation, and social perceptiveness.
Responsibilities
  • Plan, organize, and conduct occupational therapy programs to facilitate the development and treatment of injured, ill, or disabled patients through therapeutic everyday activities.
  • Assist patients in developing, recovering, improving, and maintaining skills needed for daily living and work.
  • Address patient performance aspects to support engagement in occupations affecting health, well-being, and quality of life.
  • Select and adapt the appropriate case-specific occupational therapy treatment model, method, and approach to direct interventions.
  • Apply interventions to maximize safety and performance in activities of daily living and instrumental activities of daily living.
  • Teach patients new ways to approach tasks and break activities into achievable components.
  • Assess patients' homes and other environments and advise on alterations consistent with their needs and capabilities.
  • Recommend adaptive equipment and train patients in its use.
  • Guide and educate family members and caregivers.
  • Observe, document, and report progress toward long- and short-term treatment goals.
  • Collect data and document the processes followed.
  • Document patient care services according to approved medical and facility documentation procedures.
  • Develop an appropriate in-home care plan for patients following completion of their Plan of Treatment.

Matt Fredericks featured as Senior Dining Association "member on the Move" Congratulations to Matt Fredericks, Director of Culinary Experience for Sunnyside Communities, who was recently featured by the Senior Dining Association as its "Member on the Move." The feature highlights Matt's career journey, leadership philosophy and expanded role supporting dining operations across Sunnyside, King's Grant and Summit Square. It also recognizes his commitment to developing team members, strengthening culinary operations and creating meaningful dining experiences that support resident wellness, connection and quality of life. Matt joined Sunnyside Communities in 2021 and has continued to grow as a leader within the organization.
