  • May need to assist with evacuating residents during emergencies.
Responsibilities
  • Identify residents using a wristband or photo card file before providing treatments or serving meals, as necessary.
  • Use only facility-authorized abbreviations when documenting information.
  • Report changes in residents’ conditions to the Nurse Supervisor or Charge Nurse as soon as practical.
  • Record entries on flow sheets, notes, and charts in an informative, descriptive manner.
  • Report accidents and incidents observed during the shift on which they occur.
  • Protect residents’ protected health information and promptly report suspected or known disclosure violations to the Administrator.
  • Report known or suspected unauthorized attempts to access the facility’s information system.
  • Perform only nursing care procedures for which training has been provided.
  • Prepare residents’ rooms for arrival, including making beds, displaying name tags, and ensuring admission kits are available.
  • Greet residents and escort them to their rooms; introduce them to roommates, other residents, and personnel as appropriate; and help them feel comfortable.
  • Inventory and mark residents’ personal possessions as instructed and store their clothing.
  • Assist residents with packing possessions when moving rooms or being discharged, transport them to new rooms or receiving areas, and assist with loading and unloading them from vehicles as necessary.
  • Maintain a warm, calm environment with personal interest and positive emphasis throughout the unit and shift.
  • Meet regularly with nursing personnel on the shift to identify and correct problem areas and improve services.
  • Report resident complaints and grievances.
  • Participate in and receive nursing reports as instructed, including upon reporting for duty.
  • Follow established policies concerning exposure to blood and body fluids.
  • Make beds and add extra covers as requested.
  • Measure and record residents’ temperatures, pulses, and respirations; weigh and measure residents as instructed.
  • Frequently check residents who cannot call for help, answer resident calls promptly, and routinely check that personal care needs are met.
  • Help residents identify food arrangements when needed, including describing the foods on their trays and their locations or temperatures.
  • Record residents’ food and fluid intake and report changes in eating habits to the Nurse Supervisor or Charge Nurse.
  • Keep residents’ water pitchers clean, filled with fresh water on each shift, and within reach.
  • Provide after-meal care, including removing trays and cleaning residents’ hands, faces, and clothing.
  • Complete assigned tasks according to established policies and procedures, work assignments, and schedules.
  • Cooperate with personnel in other departments and facility staff to maintain adequate nursing services for residents.
  • Notify the facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals according to facility accident and incident procedures.
  • Assist residents with daily functions, including dental and mouth care, bathing, hair combing, and dressing or undressing as needed.
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests, including laboratory work, X-rays, therapy, and dental procedures.
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Routinely check that residents’ personal care needs are met in accordance with their wishes.
  • Observe and report pressure areas and skin breakdown to help prevent bedsores.
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents and record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor or Charge Nurse if a resident leaves or is missing from the facility.
  • Use only equipment for which training has been provided and report defective equipment to the Nurse Supervisor or Charge Nurse.
  • Maintain confidentiality of resident care information, including protected health information, and report suspected unauthorized disclosures.
  • Report allegations of resident abuse or misappropriation of resident property.
  • Honor residents’ requests to refuse treatment and report those requests to a supervisor.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
