- A master's or doctoral degree from an accredited Physical Therapy program is required.
- A current state license as a Physical Therapist is required.
- At least one year of experience as a licensed Physical Therapist is required.
- Reliable transportation, a valid driver's license, and valid automobile insurance coverage are required.
- Willingness to travel daily for home visits within the service area is required.
- Strong clinical assessment and treatment planning abilities are required.
- Knowledge of medical terminology and home health regulations is required.

Responsibilities
- Provide one-on-one rehabilitation care in patients’ homes to support recovery from surgery, injury, or illness, improve function, reduce pain, and support independence.
- Assess each patient’s functional abilities and home environment.
- Develop individualized treatment plans and guide patients through targeted interventions to improve mobility, balance, strength, and safety.
- Work closely with an interdisciplinary care team.
- Educate and empower patients and caregivers.
- Help individuals make progress toward their goals and improved quality of life.
Desired Qualifications
- Home health experience is preferred.
- OASIS certification, or willingness to obtain it, is preferred.
- Experience with Medicare home health documentation is preferred.
