Requirements
  • A minimum of 2 years of experience working in educational, social service, juvenile justice, and/or mental health settings providing counseling, training, therapy, and/or guidance to transitional-age youth and their families with a bachelor's degree, or 1 year of such experience with a master's degree.
  • One year of experience working in a strengths-based model serving transitional-age youth as a facilitator.
  • A high school diploma or equivalent is required; substantial experience may substitute for education.
  • A valid driver's license, adequate personal-vehicle insurance when applicable, a safe driving record, at least 21 years of age, at least one year holding a valid U.S. driver's license, and successful completion of a driver-screening background check are required.
  • Ability to work flexible hours, including evenings and weekends, and provide rotating on-call coverage.
  • Knowledge of engagement strategies effective with transitional-age youth.
  • Competency in evidence-informed practices, techniques, and modalities.
  • Knowledge and use of advocacy techniques and different communication styles.
  • Ability to establish supportive, trusting relationships with transitional-age youth and respect their rights and personal preferences in support-plan design.
  • Knowledge of human, legal, civil-rights, community, and other resources.
  • Knowledge and use of formal and informal assessment tools and practices.
  • Ability to work independently and as part of a multidisciplinary team.
  • Competency in crisis-intervention techniques.
  • Knowledge of or ability to obtain knowledge in counseling, motivational interviewing, and teaching modalities.
  • Knowledge of home-, school-, and field-based service delivery.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Completion of New Employee Training and TIP Model Core Competency Training within one month of hire.
  • Ability to perform physically active work, including standing, walking, bending, kneeling, stooping, crouching, crawling, climbing, lifting at least 25 pounds, assisting clients with daily-living tasks, and maneuvering through narrow spaces.
  • Ability to operate a computer and office equipment and to communicate with clients, staff, and external representatives.
Responsibilities
  • Conduct strength-based needs assessments using the Strength Discovery Assessment process across employment and career, educational opportunities, living situation, personal effectiveness and wellbeing, and community-life functioning domains.
  • Coach and support 12 to 15 transitional-age youth as they progress through active, maintenance, and follow-along TIP support statuses.
  • Deliver evidence-informed or evidence-based practices as supported by the TIP Program Director and Area Clinical Director.
  • Participate in program or Access Center activities that support youth participation, advocacy, and skill and knowledge development.
  • Collect and record data on youth progress across their goals during active, maintenance, and follow-along statuses.
  • Facilitate person-centered planning and development of each youth's individualized TIP Core Competency Plan.
  • Collaborate with teachers and school guidance counselors to develop person-centered plans for high-school students and attend individualized education program meetings as a youth advocate.
  • Develop prevention and risk/crisis plans as backup support for youth who require them.
  • Collaborate with service practitioners and educators from children's mental health, education, adult mental health, child welfare, and rehabilitation to ensure appropriate services.
  • Conduct, arrange, or broker necessary services and supports in accordance with each youth's person-centered TIP Core Competency Plan.
  • Promote parent involvement and participation by other key individuals in planning services and supports.
  • Interface with adult mental health case managers to ensure continuity of age-appropriate services as individuals approach and pass age 18.
  • Help youth identify obstacles to independent living, employment, post-secondary education, and supportive social and recreational activities.
  • Develop and implement strategies to overcome identified barriers.
  • Collaborate with relevant public and private agencies to implement agreed-upon strategies.
  • Develop interagency and community-network relationships to improve access to relevant community services and benefits.
  • Refer youth to appropriate clinical, medical, and behavioral specialists when requested.
  • Chart and record all contacts and services in accordance with Vinfen and applicable funding or regulatory standards and maintain transition records.
  • Help youth access community settings and resources such as employment, housing, and Social Security benefits.
  • Manage program resources in accordance with agency regulations.
  • Perform other related duties as assigned.
Desired Qualifications
  • A bachelor's degree in the social sciences, rehabilitation, and/or education fields, including special education, psychology, or social work.
  • Experience working with transitional-age youth with emotional or behavioral dysregulation or psychiatric illness.
