Requirements
  • Graduation from an accredited school of nursing and a Bachelor’s degree in Nursing (BSN).
  • A valid Florida Registered Nurse license is required.
  • Basic Life Support (BLS) certification from the American Heart Association is required.
  • A minimum of 2 years of relevant experience is required, including a minimum of 1 year in hospital case management or nursing.
  • Working knowledge of patient assessment and medical terminology is required.
  • Ability to communicate effectively in oral and written form.
  • Ability to recognize, analyze, and solve a variety of problems.
  • Ability to analyze, organize, and prioritize work under pressure while meeting deadlines.
  • Ability to maintain effective interpersonal relationships.
Responsibilities
  • Coordinate the overall interdisciplinary plan of care for patients from admission to discharge.
  • Monitor care and serve as a liaison between patients and families, healthcare personnel, and insurers.
  • Evaluate patient needs and available resources, and recommend and facilitate the plan for the best outcome.
  • Develop discharge plans that provide available resources to meet ongoing patient needs and encourage compliance with medical advice.
  • Identify patient care issues and suggest revisions to or new clinical pathways to improve quality of care.
  • Identify patients’ risk factors or obstacles to care, including discharge and readmission risk.
  • Evaluate the plan of care regularly through chart review and patient interviews, and collaborate with the medical team to facilitate patients’ movement through the system.
  • Educate patients and families on the progression of care.
  • Serve as a liaison between patients, families, and healthcare personnel to ensure necessary care is provided promptly, effectively, and in a fiscally responsible manner.
  • Promote quality care to ensure patients receive medically appropriate services in the appropriate status and stay standards.
  • Facilitate ongoing insurance authorization for continued stay.
  • Facilitate regulatory notifications and obtain patient signatures according to policy.
  • Maintain knowledge of insurance reimbursement policies.
  • Plan and facilitate discharge and transition plans based on patient needs, ensuring they address the patient’s physical, social, and emotional needs.
  • Adhere to University and unit-level policies and procedures and safeguard University assets.
