Requirements
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • A current automobile insurance policy and valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice services, interdisciplinary hospice roles, community resources, general nursing practice, managed care principles, and applicable regulatory guidelines is required.
  • The ability to perform patient assessments, maintain clinical documentation, operate nursing assessment tools and office equipment, communicate effectively, and maintain transportation for work is required.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care based on individualized plans of care in patients’ homes, assisted living facilities, or other settings.
  • Perform comprehensive initial and ongoing patient assessments and reassessments, including in-person and telephonic assessments or screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Coordinate care with interdisciplinary teams and consult with physicians, nurse practitioners, primary care providers, and hospice physicians.
  • Provide education to patients, families, and caregivers regarding disease processes, self-care, end-of-life care, medications, nutrition, and dietary needs.
  • Communicate changes in condition and signs or symptoms of decline to the appropriate provider.
  • Support continuity of care, referrals, and transitions across home health, palliative, hospice, and other levels of care.
  • Document screening and assessment findings, physician orders, progress notes, and care plans in a timely manner.
  • Provide emotional, physical, and spiritual support, including bereavement guidance, to patients and their loved ones.
  • Teach caregivers safe care techniques and provide practical and emotional support.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the care plan requires broader participation.
  • Participate in on-call and patient support activities and conduct on-call services in a clinically competent and responsive manner.
  • Follow infection-control, regulatory, compliance, quality-improvement, scheduling, and organizational policy standards.
