Requirements
  • LPN licensure in the state of practice.
  • Current active nursing licensure must be maintained in accordance with applicable state and federal rules and regulations.
  • Must be CPR certified.
  • Must be able to lift 30 pounds and push or pull 50 pounds.
  • Must be able to stand, walk, use hands to handle objects, sit, reach, stoop, kneel, crouch, crawl, and occasionally climb or balance.
  • Must have speech clarity, speech recognition, written expression, and required sensory abilities.
  • Must be willing to work beyond normal working hours and on weekends and holidays when necessary.
  • Must work safely around fumes, airborne particles, toxic or caustic chemicals, infectious waste, diseases, and other workplace hazards.
Responsibilities
  • Perform nursing functions within the scope of practice and provide or coordinate care and services for residents' physical, psychosocial, and functional needs.
  • Practice nursing care in accordance with the state Nurse Practice Act and organizational clinical and administrative policies and procedures.
  • Collaborate with the interdisciplinary team to define individualized plans of care for each resident.
  • Work with nursing leadership to support regulatory compliance.
  • Promote individual and team learning and provide teaching and role modeling of professional nursing practices to staff.
  • Pass medications and oversee the medication ordering and administration process.
  • Administer medications and treatments to residents and monitor ordered intravenous fluids, supplemental nourishment, and medications.
  • Initiate intravenous therapy when IV-certified.
  • Maintain knowledge of observation, assessment, and documentation processes, including care and service plans and the Minimum Data Set as applicable.
  • Use the electronic medical record to capture complete clinical documentation.
  • Observe resident status to assure quality care and coordinate with resident services to maintain continuity of care.
  • Document medical information and maintain detailed, timely, and accurate reports.
  • Assess potential hazards and implement practices that maintain a safe, comfortable, and functional environment.
  • Oversee and direct certified staff and others as assigned to ensure standards of care and services are provided.
  • Report and support investigative protocols related to staff or resident incidents and accidents in accordance with state and federal regulations.
Desired Qualifications
  • Specialized training or experience in gerontological nursing is preferred.
