Requirements
  • Valid and active Registered Nurse (RN) license in the State of Connecticut
  • Diploma from an accredited nursing program
  • New graduates welcome
Responsibilities
  • Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed
  • Develops, implements, manages patient’s plan of care
  • Making revisions and documenting changes as needed
  • Actively coordinates the care of the client through communication and written documentation to the physician, and other disciplines involved in the client’s care and community resources
  • Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to appropriate resources
  • Reviews the caregiver notes daily and appropriately addresses changes in the consumer’s condition or environment
  • Ensure continuous compliance with all regulatory standards as well as company policies and procedures
  • Performs other job-related duties as may be required or directed by the supervisor
