Requirements
  • Licensure is required in social work, nursing, geriatrics, mental health, or a related field.
  • Have 3-5 years of experience working with older adults and families.
  • Have excellent clinical skills and knowledge of family dynamics and family systems.
  • Be highly organized and able to work in a non-traditional, highly professional work environment while managing a fee-for-service caseload effectively.
  • Have outstanding interpersonal and communication skills.
  • Be comfortable using electronic devices, including a smartphone and laptop or desktop computer.
  • Be able to drive and have access to a vehicle.
  • Obtain and/or maintain membership in the Aging Life Care Association.
  • Obtain and maintain certification as required by the Aging Life Care Association.
  • Have strong verbal and written communication skills in English.
Responsibilities
  • Conduct comprehensive assessments of clients’ functional, cognitive, medical, social, and emotional needs, including evaluating their health, emotional state, finances, and legal needs and identifying actual and potential concerns.
  • Assist clients and their care teams with decisions about aging, disability, services, and long-term care planning.
  • Create goal-oriented care plans to address client concerns.
  • Respond quickly to emergencies and high-risk situations.
  • Present recommendations to clients and care teams and involve all relevant parties in decision-making.
  • Implement and maintain care plans, address obstacles, and update goals as needed.
  • Perform periodic reassessments to ensure care plans meet goals and adjust plans based on feedback.
  • Coordinate care, arrange services, and make referrals for clients.
  • Facilitate communication between clients and care teams, advocate for clients, and coordinate care across doctors and specialists into a seamless plan.
  • Guide families from crisis to proactive planning for transitions or medical changes.
  • Navigate complex family dynamics while empowering older adults and their care teams.
  • Complete documentation promptly according to Honora’s service-level agreements.
  • Follow client lifecycle processes and improve workflows.
  • Proactively expand skills, stay updated on elder-care trends, and understand Honora’s services and client needs.
Desired Qualifications
  • An advanced degree in social work, nursing, geriatrics, mental health, or a related field is preferred.

Alder, an e-commerce business, focuses on clothing for women of all body types and outdoor recreation. With this investment, alder will expand its collection, launch select pop up shops across North America, add new hires and assort in REI stores this year.
