Requirements
  • One to three months of experience in a human services setting, although academic training may substitute in some cases.
  • A high school diploma or equivalent is required.
  • A valid driver's license, adequate personal-vehicle insurance when applicable, and a safe driving record are required. The employee must be at least 21 years old, have held a valid United States driver's license for at least six months, and pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, and assist persons served with daily-living tasks.
  • Ability to operate a computer and office equipment, including a calculator, copier, and printer.
  • Ability to sit, reach, climb stairs, maneuver through narrow spaces, bend, reach, file, stand, and move around the facility.
  • Ability to speak, hear, and communicate with persons served, staff, and external representatives.
  • Required vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Program certification is required within 150 days of hire.
  • Knowledge and use of advocacy techniques, different communication styles, empowerment techniques, self-advocacy techniques, personal-management skills, household-management skills, and crisis-intervention techniques.
  • Ability to document interventions, work independently and as part of an interdisciplinary team, teach skills, and provide role-model behaviors.
Responsibilities
  • Assist in developing, implementing, and documenting individual treatment support plans.
  • Perform daily tasks with persons served, including rehabilitative and behavioral interventions, appointment scheduling, progress monitoring, transportation, and related support.
  • Provide role-model support and assist persons served in becoming integrated into their communities.
  • Assess the physical and emotional status of persons served and report changes to a supervisor.
  • Ensure program-site safety and maintenance, and oversee and participate in daily living and cleaning activities such as cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Comply with corporate and divisional policies and procedures, including safe work practices, workers' compensation, universal precautions, and exposure-control procedures.
  • Store, administer, document, and dispose of medication for persons served in accordance with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Complete paperwork and logs, including communication logs, community-membership logs, chore schedules, timesheets, and application forms.
  • Perform crisis intervention as needed.
  • Provide current contact information to the Relief Pool Coordinator or Human Resources staff.
  • Perform other related duties as required.
