  • Be a member in good standing of the OIIQ or OIIAQ.
  • A DEC diploma is required.
  • A criminal record check (plumitif) is required.
  • A valid driver's license is required.
  • Current CPR certification is required.
  • A curriculum vitae is required.
  • A car is mandatory for this assignment.
Responsibilities
  • Assess the physical and mental health condition of clients in their homes.
  • Plan, provide, and follow up on nursing care.
  • Develop, implement, and adjust the nursing therapeutic plan (PTI) and intervention plan.
  • Provide education to clients and their relatives.
  • Identify situations involving risk and intervene according to current protocols.
  • Collaborate with the interdisciplinary team and network partners.
  • Contribute to coordinating and organizing services.
  • Provide home palliative care to clients as needed.
