Requirements
An active registered nurse state license is required.
At least two years of nursing experience are required.
At least two years of home health nursing experience are required.
A valid driver's license is required.
Responsibilities
Organize, develop, manage, and direct the home health functions of the community health hub.
Manage a team of clinicians to ensure patients receive exceptional care in their homes.
Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs.
Conduct patient visits as needed.
Manage emergency situations promptly, precisely, and professionally.
Complete medication administration and treatment needs for assigned patients.
Provide nursing care in accordance with applicable regulations and company policy.
Prepare and maintain accurate and complete clinical and progress notes in a timely manner.
Desired Qualifications
Supervisory experience is preferred.

Responsibilities
What You Will Do:

Complete medication administration and treatment needs for all assigned patients
Provide nursing care in accordance with applicable regulations and Company policy
Prepare and maintain accurate and complete clinical and progress notes on a timely basis
Qualifications
Qualifications:
Active RN state license
Two or more years of nursing experience (supervisory experience preferred)
Two or more years of home health nursing experience required
Valid drivers’ license
