Requirements
  • Employment eligibility verification and authorization to work for any employer in the United States.
  • Ability to obtain CPR/First Aid training.
  • Successful completion of a tuberculosis test, background screening, and drug test as conditions of hire.
  • Reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
  • Ability to work in California and perform regular and customary work within the state.
Responsibilities
  • Conduct intake and individualized needs assessments for all clients and collaborate to develop an Individualized Service Plan addressing housing goals, income, medical, mental health, substance use, financial, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their Individualized Service Plan goals.
  • Complete program-specific assessments, including the VI-SPDAT.
  • Evaluate strengths and challenges related to short-term and long-term goals and conduct a 90-day review with each client after initial intake.
  • Maintain documentation standards required by the program and PATH policies, complete case file notes in a timely manner, and maintain complete, accurate, and current client files.
  • Maintain the confidentiality of client files in accordance with HIPAA and applicable guidelines.
Desired Qualifications
  • Two years of experience in healthcare, case management, homeless services, working with vulnerable populations, or a related field.
  • Ability to gather and interpret data, identify trends and patterns, and produce accurate work.
  • Ability to collaborate effectively with colleagues, participants, and stakeholders.
  • Ability to communicate clearly and effectively verbally and in writing to individuals and groups.
  • Ability to take initiative, make decisions independently, and develop solutions to problems.
  • Ability to analyze and evaluate information, identify key issues, consider alternatives, and make logical decisions.
  • Ability to maintain ethical and professional standards and appropriate boundaries.

PATH is a California nonprofit network that fights homelessness by providing permanent housing with supportive services. It uses a Housing First approach to stabilize people by prioritizing long-term housing over temporary fixes. The organization operates through a family of agencies offering street outreach, interim and permanent housing, rapid re-housing, housing navigation, eviction prevention, employment services, veterans programs, and coordinated health care, plus PATH Ventures which develops affordable housing and reinvests profits into mission work. It funds its work through foundations, corporations, individual donors, and government partnerships, with a goal of expanding access to stable housing and wraparound services to end homelessness.

Among this group, engagement is current: 28.3% are presently in therapy, and another 25% have sought therapy within the past year.
