  • A high school diploma or GED is required.
  • Must meet Center for Medicaid/Medicare Services-approved state and/or national certification requirements within the required state or CMS timeline.

  • The position requires travel between assigned facilities and various locations within the community; travel to regional, Business Unit, and Corporate meetings may be required.
  • The role requires frequent, prolonged standing and bending, assisting with lifting and moving patients and ambulation, and lifting solutions up to 30 pounds frequently and up to 40 pounds occasionally; heavier items are supported by two-person assistance and material-assist devices.
  • Previous patient care experience in a hospital setting or related facility is preferred.
  • Continued employment depends on successful completion of the organization's dialysis training program and CPR certification.
Responsibilities
  • Provide safe and effective dialysis therapy as part of the dialysis health care team, under the direct supervision of a licensed nurse and in accordance with organizational policies, procedures, training, and applicable regulations.
  • Set up and operate hemodialysis machines, maintain a safe and clean work environment, and participate in Quality Enhancement Program and Continuous Quality Improvement activities, including patient-satisfaction and process-improvement activities.
  • Assist healthcare team members in providing patient-specific education about adequacy measures, including Online Clearance Monitoring, Adequacy Monitoring Program, Urea Kinetic Modeling, disease processes, and vascular access, where appropriate.
  • Welcome assigned patients and ask about their wellbeing since their previous treatment.
  • Evaluate vascular access for patency, perform vascular access cannulation, and administer heparin as delegated or permitted by state law.
  • Obtain pre- and post-treatment vital signs and weight, and evaluate vascular access before treatment.
  • Initiate dialysis according to prescribed orders, including blood flow and dialysate flow; evaluate intradialytic problems and intervene as prescribed or directed; and monitor patients' responses to dialysis therapy.
  • Obtain vital signs before reinfusion, perform and document functions required to discontinue treatment, and evaluate patients before termination of venous access, including standing and sitting blood pressure.
  • Obtain hemostasis and apply appropriate dressings, then evaluate patients before discharge.
  • Perform and record pre- and post-dialysis evaluations, weights, and vital signs with initial identification.
  • Calculate and enter each patient's prescribed dialysis machine programming for ultrafiltration goal, treatment time, and ultrafiltration modeling.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits 160.
  • Record accurate and timely vital signs and treatment parameters on the Hemodialysis Treatment Sheet with initial identification, and complete required treatment documentation and progress notes where applicable.
  • Report significant information and changes in patient condition to the registered nurse or supervisor; observe patients and conduct machine safety checks according to facility policy, reporting unusual findings.
  • Perform and document interventions for unusual patient status and document patient responses; monitor vascular access pressures during treatment, intervene as needed, and document and report unusual findings.
  • Obtain prescribed laboratory tests and prepare specimens for collection.
  • Clean blood spills immediately with appropriate disinfectant according to facility policy; organize and use supplies and equipment efficiently to prevent waste.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist in training applicable direct patient-care staff on appropriate programs under nurse supervision.
  • Follow organizational business policies, procedures, and systems incorporated into training, including ethical business practices.
  • Maintain environmental integrity and aesthetics by ensuring all areas are safe and clean; clean and disinfect dialysis machine surfaces, chairs, equipment, and surrounding areas between treatments according to facility policy.
  • Prepare for dialysis treatment, document as appropriate, perform required pretreatment machine alarm tests including Pressure Holding Test, and initiate the Solution Delivery System.
  • Complete and document ongoing participation in Continuous Quality Improvement activities and enter treatment data accurately and promptly into the designated clinical application.
  • Review treatment sheets for completeness, verify nursing signatures, and ensure omitted entries are completed or corrected by appropriate staff.
  • Prepare initial patient charts for admission and compile paperwork for placement in the chart.
  • Prepare and print laboratory requisitions for prescribed or stat specimens according to laboratory destination; collect, label, prepare, and store samples according to laboratory specifications; and ensure appropriate laboratory courier collection.
  • Assist with collecting information for infection-control audits, support staff and patient adherence to infection-control practices, and follow infection-control policies and procedures.
  • Participate in and reinforce infection-control education for patients and families under the direction of the Center Manager or designee, as allowed by state law.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate their transitional dialysis care.
  • Reinforce education and care matters for new patients as allowed by state law.
  • Liaise with appropriate staff to support patient experience and ensure disciplines are scheduled according to the care plan.
  • Help patients understand home dialysis products, the benefits of home dialysis, and how it may work for them.
  • Set up and perform treatments on all machines used in the transitional care unit.
  • Help patients understand treatment options, including demonstrating peritoneal dialysis setup, and assist with coordinating transitions to the patient's chosen modality.
  • Assist with supply ordering and inventory for the transitional care unit.
