Requirements
  • Practices within the Licensed Practical Nurse scope of practice.
  • Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of residents before and after procedures, geriatric competency, hospice care, pain management, resident safety, restraints, isolation precautions and personal protective equipment, and admission and discharge of patients or residents.
  • Provides written and verbal educational material to residents, patients and families, and documents education according to policy.
  • Accurately transcribes and documents physician orders using a computerized order-entry system.
  • Accurately reconciles medications upon resident admission and discharge.
Responsibilities
  • Administers routine services in caring for sub-acute, chronic and convalescent adult and geriatric residents under the guidance and supervision of a Registered Nurse.
  • Identifies and reports changes in resident status to the Registered Nurse in a timely manner.
  • Updates and implements resident plans of care appropriately.
  • Participates in resident feeding and oversees dining areas as assigned.
  • Aids in the orientation of new employees.
